Position Overview:

Coordinates the resources and services of pulmonary rehabilitation (PR) program including acute patients. The goal is to help restore pulmonary patients to an optimal level of function while evaluating post-acute follow-up and resources. Provides age-based care relevant to the development/psychosocial needs, environmental safety, educational needs, communication style, and level of understanding for each patient population—pediatric, adolescent, geriatric. Works to reduce the readmission of patient populations with PR qualifying diagnoses (e.g., chronic obstructive pulmonary disease (COPD)) by providing transitional support, follow-up and timely access to the PR Program, and visibility to community resources/agencies. Offers oversight for the PR Program to ensure adequate staffing and provide the highest quality of patient care. Cardiovascular coordinators review all admits for acute/chronic heart failure and acute myocardial infarction and participant in multidisciplinary rounds. Coordinates with case management regarding access to follow-up and medications along with primary care physician care and specialist referrals. Collects readmission data and presents this data to the interdisciplinary care team relative to patient volumes and outcomes.

• Orients new referrals to the PR program and CVC, including assessing the risk factors and function, establishing patient goals and instruction on self-monitoring techniques.

• Provides education on the age-related needs of patients with consideration for their developmental/psychosocial needs, environment safety, educational need, communication style, and level of understanding.

• Supports and meets with hospital patients who could benefit from available transitional and outpatient programs to develop a plan, using shared decision-making and teach back/show me methods. Participates in MDR.

• Oversees and coordinates process improvement activities related to the organization-wide outcome measures defined through established quality and safety strategic goals which optimize clinical care and considers readmission reduction and goals of care.

• Collects and retrieves data, designs, and uses spreadsheets and databases for clinical and administrative decision making, and analyses data for relationship to outcome. Presents data to key stakeholders for performance and outcomes.

• Assists in the development of an organization wide performance-improvement plan which integrates the medical staff/medical group structure and activities, and the development of long-term strategies and methodologies for evaluation of process and outcomes across the continuum of care.

• Records and tracks, and analyzes accurate, complete data needed for statistical reporting, patient survey, etc. to improve the program/protocols.

Job Description :

EDUCATION:

  • Graduate of Respiratory Care Program accredited by the Committee on Accreditation for Respiratory Care (CoARC).

  • Bachelor’s: Management, public health, nursing, business administration, social work organizational leadership or related field.

CERTIFICATION & LICENSURE:

  • BLS-Basic Life Support Healthcare Provider

  • OR ACLS-Advanced Cardiac Life Support

CERTIFICATION & LICENSURE

  • CA Licensed Respiratory Care Practitioner (RCP)

  • For CA Licensed RCPs, Registered Respiratory Therapist (RRT) certification in addition is preferred

OR

  • RN-Registered Nurse of California

  • Or LCSW-Licensed Clinical Social Worker

  • Or PT-Physical Therapist

  • Or OT-Occupational Therapist

TYPICAL EXPERIENCE:

  • 5 years of recent relevant experience.

SKILLS AND KNOWLEDGE:

  • Demonstrated expertise in pulmonary mechanics and patient education in the pulmonary system, including a general understanding of anatomy, physiology, pathophysiology, chemistry, and physics.

  • Working knowledge of applicable local/state/federal regulations and guidelines related to respiratory therapy and cardiopulmonary. Familiarity with the current transition and outpatient support programs available to patients with respiratory conditions.

  • Solid understanding of the key risk factors of readmission and patient support methods to reduce this risk.

  • In-depth knowledge of hospital organizational structure and operations, and experience working collaboratively with hospital staff and physicians.

  • Exceptional attention to detail with solid analytical and project management skills with proficiency in program development and public relations.

  • Possess written and verbal communications skills to explain complex clinical information and program data clearly and professionally to diverse audiences.

  • Prioritize and delegate assignments under varied and urgent conditions and work based on experience, procedures, policies, and best practices to achieve objectives and meet deadlines.

  • Lead others to solve complex problems using sophisticated analytics to identify innovative solutions.

  • Influence individuals or groups with different opinions and to enlist cooperation without direct control or authority.

  • Analyze problems, reach practical conclusions, and institute effective changes.

  • Ensure the privacy of each patient’s protected health information (PHI).

Pay Range is $57.77 to $72.21 / hour

The salary range for this role may vary above or below the posted range as determined by location. This range has not been adjusted for any specific geographic differential applicable by area where the position may be filled. Compensation takes into account several factors including but not limited to a candidate’s experience, education, skills, licensure and certifications, department equity, training and organizational needs.
