Reasons Eating Disorder Center (Reasons) is seeking candidate professionals, preferably with previous experience working with eating disorders, trauma, substance use and LGBTQIA+ communities, to provide compassionate care to our diverse patients for our residential treatment facilities located in beautiful, intimate six-bed houses in the Pasadena, San Marino, and the Arcadia neighborhoods of the Los Angeles area.

The Case Manager for Reasons Residential is responsible for managing the social work and discharge planning coordination offered to the Reasons patients, which may include the appropriate clinical screening and assessment of new admissions, input into the establishment of treatment plans, interfacing with the treatment team/external case managers, authorizations, and utilization review with managed care organizations. This position provides supervision and/or implements and accomplishes all aspects of the discharge planning process. This position will provide aftercare planning, relapse prevention, and life skills-oriented therapy groups for patients at each facility. This position is a key member of the team and will travel between 3 residential sites in the San Gabriel Valley in Pasadena, Arcadia and San Marino and attend treatment teams, audit medical records, meet with patients to coordinate discharge planning and social work needs.

Requirements:

Education: R.N. or Masters or Doctorate Degree from an accredited college or university in social work, mental health, psychology or a related field required.

Experience: A minimum of four (4) years direct clinical experience in a psychiatric or mental health setting preferred. Experience in patient assessment, family motivation, treatment planning and communication with external review organizations or comparable license. One (1) year of experience working in an eating disorders treatment setting preferred.
