HCR Home Care Physical Therapist - Full Time - Monroe County in Rochester, New York

Role and Responsibilities

A qualified professional, who directs, supervises, evaluates and provides physical therapy services to patients in the home , following physicians orders.

Essential Functions

  • Evaluate patient using diagnostic muscle, nerve, joint,and functional abilitytestsand measures.

  • Prepare an assessment of the patients condition at the time of theinitialevaluation and perform periodic re-evaluations, develop a written problem list and plan of care including types and frequencies of treatment.

  • Recommend modification of plan of careto physician,as needed.

  • Document clinical notes on the date of the visit into medical record and keep all records and summariesup-to-date.

  • Treat patients to address impairments and functional limitations, with a goal ofreachingmaximum functional independence.

  • Instruct and/or recommend patients and their caregivers in the use of prosthetic, orthotic,and durable medical equipment (canes, walkers, wheelchairs, etc.).

  • Include and educate thepatientandcaregiver(s)regardingtreatmentplanand goals.

  • Attend physical therapy staff meetings and in-service presentations.

  • Participate in case conferences.

  • Participate in the development and periodic revision of the physicians Plan of Treatment.

  • Assureappropriate safetypractices and maintenance of equipment.

  • Other duties as assigned.

    Education Requirements

  • Graduateof an accreditedPhysicalTherapy program.

    Qualifications and Requirements

  • Current, valid Physical Therapy licenseissuedbyNYSDepartment ofEducation.

  • One year of practical experience as a licensed physical therapist.

    Work Environment

    The Physical Therapist is primarily in a non-office setting and may be exposed to outdoor conditions.

    The working conditions are classified as medium work:

  • Medium work - Exerting up to 50 pounds of force occasionally, and/or a negligible amount of forcefrequentlyor constantly to lift, carry, push, pull or otherwise move objects, including the human body. Medium work involves sitting and standing.

    Physical Requirements
