University of Rochester Pharmacist, Pending Licensure in Rochester, New York

GENERAL PURPOSE:

This job classification is intended to be used on a temporary basis for the employment of a staff member who has completed all educational requirements and has successfully graduated from an accredited school of pharmacy, but is awaiting final licensure and registration as a pharmacist in NYS (this person is not currently licensed in another US jurisdiction). Under the direct supervision of a registered pharmacist, a Pharmacist Pending Licensure with a valid NYS Intern permit may perform the functions of a Pharmacist. These functions include preparing, compounding, and dispensing prescriptions and medications for patients of SMH (Neonate, Pediatric, Adult, Geriatric) in accordance with all applicable state and federal laws and regulations for Pharmacy Practice. Employees within this classification are expected to sit for their licensure exams at the earliest possible time. Employees have a maximum of 1 year to obtain their Pharmacist License from NYS.

JOB DUTIES AND RESPONSIBILITIES:

  1. Read and follow departmental and hospital policy and procedures related to dispensing of medications. Dispense medications without error and resolve problem medication orders appropriately and in a timely manner. Check all products prior to dispensing as required

  2. Process medication orders utilizing current information system technology to create and maintain patient profiles.

  3. Document clinical activities and interventions on a regular basis

  4. Direct work of technicians to ensure appropriate workflow as required.

  5. Develop or participate in clinical practice by rounding with physicians, nurses, and other health care professionals on patient care units, as assigned.

  6. Monitor assigned patient populations for clinical progress including renal/hepatic function, culture results, and other applicable laboratory tests. Recommend adjustments to therapy as necessary.

  7. Identify the need for patient teaching and perform such tasks when necessary.

  8. Conduct QA, MUE, or other research to document the value and contribution of clinical pharmacy services at URMC within the area of specialization. Participate in departmental Q/A processes such as reporting and following up on medication incidents, reconciling MARs, cost-savings initiatives, etc.

  9. Resolve problems with appropriate and direct communication to all pertinent staff in a clear and concise manner. Identifies patient care problems and does not pass on problems to the next shift without exhausting reasonable attempts at resolution and communicating progress. Problem-solving will often go beyond patient-specific issues and may require participation in interdisciplinary teams or revising hospital or departmental policies, procedures, or treatment guidelines. Contact supervisor for assistance when the solution is beyond their expertise or authority.

QUALIFICATIONS:

A degree in pharmacy (BS or Pharm.D.) from an ACPE-accredited school of pharmacy required. Pharmacist licensure from NYS must be obtained within 1 year of hire. Foreign graduates must have received a determination of equivalent training and education accepted by the NYS Board of Pharmacy. The candidate must also possess a valid NYS Pharmacy Intern permit. The position requires excellent communication skills, attention to detail, and independent judgment and decision-making preferred.

Pay Range

Pay Range: $ 46,010 - $ 62,109 Annually

The referenced pay range represents the minimum and maximum compensation for this job. Individual annual salaries/hourly rates will be set within the job’s compensation range, and will be determined by considering factors including, but not limited to, market data, education, experience, qualifications, expertise of the individual, and internal equity considerations.
