Hillside Family of Agencies Permanency Specialist II in Rochester, New York

Hillside Institute for Family Connections® is currently hiring a Permanency Specialist who will be working with youth and families receiving services within Hillside’s Therapeutic Foster Care program across counties in the Western and Southern Tier of New York. This position will support the implementation of Permanency Roundtables in the Therapeutic Foster Care program. A Permanency Roundtable (PRT) is a structured youth and family consultation, which includes six phases. The purpose is to develop a strength-based plan, which expedites permanency for youth in care, as well as identify systemic barriers. Permanency Roundtables bring people together to focus solely on permanency planning for youth which includes promoting relational and legal permanency. The Permanency Roundtable process provides a forum for innovative thinking among team members to create action steps that support youth achieving permanency. This is accomplished through developing a concise action plan with responsible parties for each action step, working towards specific target dates.

This is a Full Time position - 40 hours per week.

Responsibilities include, but are not limited to:

  • Continue the partnership with Casey Family Programs as related to innovations in Permanency Roundtables.

  • Prepare and update all Permanency Roundtable Training materials.

  • Coordinate and deliver Permanency Roundtable Training.

  • Provide coaching to trained Facilitators and Scribes.

  • Monitor reporting associated with the Permanency Roundtable Workflow.

  • Act as a Facilitator and Scribe in agency Permanency Roundtables.

  • Act as an internal Permanency Consultant in agency Permanency Roundtables.

  • Coordinate Permanency Roundtables.

  • Develop the implementation of Youth Voice Permanency Roundtables within Therapeutic Foster Care

Requirements: Associates Degree in Human Services related field. An equivalent combination of education and experience will be considered for meeting the qualifications/requirements of the position. All candidates must have a valid NYS Driver’s License and must meet agency driving and insurance standards.

$19.46 Minimum pay rate, $27.75 Maximum pay rate, based on experience.
