University of Rochester Patient Transfer Coord III in Rochester, New York

GENERAL PURPOSE:

Works with ED team(s) to identify patients that are being admitted and who may be appropriate for transfer to another URMC facility for admission. Remains informed on the services offered at the various URMC sites. Discusses real time reasoning, details and benefits of transfer to another URMC affiliate site for admission with patients and their families in the ED. Works to improve the admission process and to assist with patient load balancing across the URMC system for the sake of patient safety and improved care.

JOB DUTIES AND RESPONSIBILITIES:

  1. Works with ED staff to identify patients for admission and for appropriate transfer for admission out of the ED. Discusses transfer process with patients, answers questions about affiliate sites. Helps complete necessary steps to complete transfers.

  2. Loads balancing initiatives amongst URMC sites. Develops and maintains relationships with URMC facilities to understand their capabilities with regards to patient management broadly, as well as on a day-to-day basis.

  3. Completes paperwork with patients to complete transfers, ensuring documentation for type of transportation needed is complete. Ensures tracking of transfers out of ED, including metrics such as volume, times of day, transportation wait times, hurdles encountered, etc.

  4. Maintains close communications with transfer center regarding possible and confirmed transfers and bed availability at URMC facilities.

  5. Monitors transfer processes and ED workflows around admissions for possible areas of improvement and efficiency. Collects feedback from stakeholders as necessary and assists in related QI initiatives.

  6. Maintains department and hospital expectations, including but not limited to, HIPAA compliance, EMTALA, etc.

  7. Other duties as assigned.

QUALIFICATIONS:

HS or equivalent required; Bachelor's degree preferred or equivalent combination of education and experience. Experience in customer satisfaction roles and healthcare fields preferred. Experience in inpatient or emergency medicine settings preferred. Required knowledge, skills and abilities: Medical general healthcare terminology, de-escalation skills, adept use of some technology, including translator iPads for example, excellent communication abilities across all ages and backgrounds.

Pay Range

Pay Range: $18.52 - $25.01 Hourly

The referenced pay range represents the minimum and maximum compensation for this job. Individual annual salaries/hourly rates will be set within the job’s compensation range, and will be determined by considering factors including, but not limited to, market data, education, experience, qualifications, expertise of the individual, and internal equity considerations.
