The Finger Lakes Performing Provider System (FLPPS) is a network of clinical and community-based providers working together across the 13-county Finger Lakes region to transform healthcare delivery, advance system transformation, close gaps in care, and support high-risk individuals and populations. FLPPS and our affiliated health home, the Greater Rochester Health Home Network (GRHHN) support providers in integrated healthcare delivery system optimization, care management, network and program management, population health and data analytics, and learning and workforce development.Position SummaryResponsible for managing FLPPS partners across the 18 county Career Pathway Training region. Provide structured support for Partner engagement and performance, data collection and consulting services to achieve planned outcomes. Collaborate with program teams, workgroups, and committees responsible for programs and initiatives.Key Accountabilities:Participate in the planning, development, and execution of partner engagement, and responsible for overall program management activities. Manage Partner Data: Oversee the collection, organization, and maintenance of all partner data, ensuring accuracy and completeness.Contact Information Management: Maintain up-to-date contact information for all partners, ensuring easy access and communication.Relationship Management: Develop and nurture relationships with partners, acting as the primary point of contact and ensuring their needs are met.Data Collection and Updates: Implement and manage processes for regular data collection and updates, ensuring timely and accurate information.Program Support: Collaborate with program teams to support the implementation of the career pathway training program, providing necessary partner data and insights.Performance Monitoring: Track and report on partner engagement and performance, identifying areas for improvement and ensuring program goals are met.Communication: Facilitate regular communication with partners through meetings, site visits, and outreach activities to support program implementation.Consulting Services: Provide consulting services to partners, assisting them with the implementation of programs and initiatives at an operational level when necessary or appropriateCollaboration: Work closely with various departments, including marketing, IT, and customer service, to ensure seamless program execution.Compliance and Reporting: Ensure compliance with all relevant regulations and reporting requirements, maintaining accurate records and documentation.Support the implementation of programs by providing project management support to partners, meeting facilitation, and other agreed upon consulting services when necessary Conduct research, analysis, and develop awareness of market/industry trends, government regulations, legislation, and guidelines; understand and interpret this information; including identifying new Partner support and business development opportunitiesCollaborate with FLPPS leadership and subject matter experts on development of necessary Partner education (e.g. webinars, workshops, etc.) to use by Program Management team Educate Program Management Team on new project initiatives and ensure appropriate talking points and Partner education materials are developed in a standardized and timely fashionRelay feedback and questions from Partners to internal teams (learning & development, analytics, care management, finance, and IT) in a timely manner and ensure they get addressed and communicated back to the Partner within a reasonable and expected time frameRequired Qualifications:Bachelor’s degree or equivalent experience requiredPMP / Project Management Professional Certification desired5+ years of project or program management experience working, preferably, within the health care or higher education sector 5+ years’ experience building and supporting client/partner relationships to meet existing needs as well as to identify and shape strategic alliances for future initiatives Understanding of, or capability to quickly learn federal and state healthcare initiatives and regulations.Experience with work plans, budgets, and performance and financial reporting.Advanced skills in Microsoft Office applications including Excel, PowerPoint, and Word.CompetenciesPossess a high level of expertise and comfort level working in ambiguous situationsExcellent analytical and listening skills with the ability to synthesize information to build project plans from beginning to endProven expertise performing as an internal and external consultantAbility to embrace a highly collaborative and frequently changing environmentTrack record includes planning, coordinating and completing projects that involve both IT systems and business process changes - demonstrating a proven ability to integrate people, process and technology to effectively meet measurable outcomesStrong sense of urgency and proven background for setting and meeting deadlines Ability to think and problem solve at a systems level, as opposed to only on a program or organizational delivery level (e.g., identify strategies focused on increasing coordination or filling gaps in service)Excellent organizational skills and attention to detail to manage multiple projects concurrentlyStrong interpersonal and communication skills and the ability to work effectively with a wide range of constituencies in a diverse community.Collaborate across stakeholder organizations and functions to ensure consistent implementation of new, improved processes and protocolsExcellent written and oral presentation skills, with the ability to engage across all levels of an organizationOutstanding discretion in handling of Protected Health Information, financial and other confidential informationConsistent demonstration of FLPPS Brand Promise: Collaborative, Trusted, Result-Oriented, Strategic and AdaptiveEssential Job Functions:In support of the American with Disabilities Act (ADA), this job description lists those responsibilities and qualifications deemed essential to the position. Minimum Salary: 80,000 Maximum Salary: 90,000 Salary Unit: Yearly
