The Supervisor Caregiver at ElderOne supervises the staff to ensure regulatory compliance and quality while maintaining an optimal level of independence for the enrolled frail elderly participants.
ELDERONE
Dedicated to providing the entire continuum of medical care and support services to seniors age 55 and older with chronic care needs. The program combines nursing, home health care, medical, transportation, rehabilitative and supportive services, personalized so that the elderly can maintain their independence without compromising their medical care or safety.
STATUS: Full Time
LOCATION: Rochester, NY
DEPARTMENT: ElderONE
SCHEDULE: Nights, every other weekend
ATTRIBUTES
- Licensed Practical Nurse, current license in the State of New York required
- 2-3 years of supervisory experience preferred
- 1 year experience working with the frail elderly
- Ability to communicate effectively
- Ability to establish and maintain cooperative working relationships
- Strong degree of confidentiality required
RESPONSIBILITIES
- Collaboration. Participates as an active member of the Management Group pursuing and supporting goals and objectives. Responsible for maintaining weekly employment hourly requirements of caregivers to ensure quality coverage for participants/residents. Works with Human Resources to recruit, interview and hire in a timely manner. Coordination of scheduling of staff for mandatory in-services and assist in coordination of staff
competencies. - Patient Care & Service. Oversee coordination of transitional housing admissions and discharges to ensure quality care
during alternative housing stays. Communicate and coordinate with Community Health Nurses the homecare concerns or issues that may need to be addressed with a home visit or family meeting.
PHYSICAL REQUIREMENTS: M - Medium Work - Exerting 20 to 50 pounds of force occasionally, and/or 10 to 25 pounds of force frequently, and/or greater than negligible up to 10 pounds of force constantly to move objects; Requires frequent walking, standing or squatting.
LICENSES/CERTIFICATIONS:
- LPN - Licensed Practical Nurse - New York State Education Department (NYSED)
The listed base pay range is a good faith representation of current potential base pay for successful applicants. It may be modified in the future. Pay is determined by factors including experience, relevant qualifications, specialty, internal equity, location, and contracts.
