University of Rochester Health Educator II in Rochester, New York

GENERAL PURPOSE:

The Tobacco Treatment Specialist is responsible for providing comprehensive education and counseling on tobacco cessation through Wilmot’s Tobacco Cessation Program. Wilmot’s Tobacco Cessation Program offers a smoking cessation text messaging program, available in English and Spanish. Moreover, the Program offers nicotine replacement therapies (e.g., nicotine patches, gum, and lozenges) at no cost. Interested individuals will conduct initial consultations, develop personalized treatment plans, and provide counseling and cessation follow up via text-messaging.

RESPONSIBILITIES:

Coordinates the implementation efforts of Wilmot’s Tobacco Cessation Program.

  • Promotes, maintains and improves individual and/or community health by assisting individuals and communities through evidence-based health information to adopt healthy behaviors- particularly via tobacco cessation.

  • Acts as primary Tobacco Treatment Specialist for the program

  • Works independently and receives minimal guidance.

  • Conducts referral follow up and initial consultations.

  • Develops evidence-based individual personalized treatment plans and provides counseling via text messaging.

  • Tracks cessation status follow up.

  • Works collaboratively with patients and provides options for change including training, pharmacologic treatment, and relapse prevention strategies.

  • Proactively builds relationships with referring providers and community organizations, and ensures service coordination and continuity of care.

Documentation and Data Reporting:

  • Documents all aspects of referrals from providers in medical records or in correspondence to providers.

  • Performs quality assurance monitoring and routine reporting.

  • Analyzes related data and identities trends, working with Program Director to provide recommendations and solutions for continuous program improvement.

Community Engagement:

  • Participates in community outreach and engagement activities that align with the mission of Wilmot’s Tobacco Cessation Program.

  • Assists in building community partnerships to enhance utilization of Wilmot’s Tobacco Cessation Program.

Maintains an in-depth knowledge of tobacco cessation expertise by reviewing literature and abstracts, attending pertinent internal and external meetings and seminars. Maintains a working knowledge on other cancer prevention and control programs available at Wilmot.

QUALIFICATIONS:

  • Bachelor’s Degree required

  • Degree in Counseling, Health Education, Nursing, Social Work, Public Health, Nutrition, preferred.

  • 2-3 years of related experience; or an equivalent combination of education and experience. Experience in health care settings preferred.

  • Fluent in English and Spanish required.

  • Certified Tobacco Treatment Specialist Training License completed upon hire (paid by University) required.

Pay Range

Pay Range: $46,987 - $65,790 Annually

The referenced pay range represents the minimum and maximum compensation for this job. Individual annual salaries/hourly rates will be set within the job’s compensation range, and will be determined by considering factors including, but not limited to, market data, education, experience, qualifications, expertise of the individual, and internal equity considerations.
