Position Description

General Description

Under general supervision from the Program Manager, the Migrant Assistance Case Manager provides case management services to eligible migrants to promote their stability and support their journey toward a regular immigrant status. The Migrant Assistance Case Manager works in coordination with other agency staff as well as community partners, volunteers, and service providers to connect migrants with appropriate services and resources.

Starting Wage: $20.50 / hour

Essential Duties and Responsibilities

  • Provides appropriate information in response to queries regarding program services.

  • Screens potential clients for program eligibility or for other programs within the community.

  • Provides intake and completes program applications.

  • Conducts needs assessment and develops a service plan with the client.

  • Provides referral to appropriate departmental, agency, or external programs.

  • Directly provides assistance to enrolled clients including counseling, advocacy, intervention, and general facilitation so that clients can access the services and resources for which they are eligible.

  • Documents service provision through case log entries, maintains client casefiles, and secures appropriate documentation per program requirements.

  • Enters and updates client and program data in the electronic data platforms related to the program.

  • Participates in regular program quality assurance practices and keeps track of quality assurance activities.

  • Assists the program in preparing for internal and funder monitoring.

  • Adheres to agency case management principles, resettlement program service delivery philosophy, and internal program manuals.

  • Assists in the preparation and submission of required reports.

  • Maintains the confidentiality of all client records per the requirements of HIPAA.

  • Participates in program development as well as in meetings and staff training activities.

  • Is aware of and actively supports the agency’s cultural competence goals, efforts, and activities.

  • Actively pursues development of professional competencies related to job role performance through reading, supervision, in-service training, and attendance at conferences and workshops.

  • Awareness and active support of the Agency's Mission, Vision, Values and Strategic Plan. Including, but not limited to, supporting the Agency's Core Competency initiatives of being Collaborative and Community Focused, Innovative and Entrepreneurial, and Culturally Competent and Diverse

  • Other duties as assigned.

Position Qualifications

Education: Bachelor’s Degree in Social Work, Human Services, or related field. Equivalent combination of education and experience will be considered

Credentials: Valid and clean NYS Driver’s License and use of a registered and reliable vehicle.

Experience: At least one year of experience in the Human Services field and/or case management experience. Experience working with volunteers, immigrants, and local service providers preferred.

Bilingual: Bilingual in English / Spanish is required.
