Reception and Placement (R&P) Resettlement Program Case Assi

Under general supervision from the Resettlement Program Manager, the Resettlement Program Case Assistant

provides case management support services for Resettlement clients. Working in concert with agency staff, volunteers

and external partners, the case assistant ensures that cases receive required services in compliance with funder

requirements as well as CCFCS case management principles. The case assistant effectively supports ongoing and

collaborative communication amongst all program staff and is responsible for ensuring a trauma informed care

environment.

Essential Duties and Responsibilities

o In close coordination with resettlement program team members assisting the Resettlement program

to ensure timely provision of all required services per the standards and requirements outlined by

funders and other governing bodies.

o Participates in preparing friends & relatives of cases to assist in the delivery of services and material

goods.

o Assists with airport reception services.

o Assists with coordinating basic needs requirements – food, clothing, home furnishings and personal

items.

o Takes part in conducting intake, orientation, and assessment of clients.

o Provide client transportation to and from various appointments.

o Assists clients with bus training.

o Facilitates and coordinates clients’ access to identified service providers in the field of health,

education, public benefits, employment, and other community partners and resources.

o Helps with documenting services in client case logs and maintaining individual case files in accordance

with funder and agency requirements.

o Assists other Department programs as needed.

o Adheres to agency case management principles and funder service delivery philosophy.

o Participates in program, departmental, and agency initiatives aimed at quality improvement and

routine program maintenance.

o Attends regular inter-departmental and other agency staff supervisory and informational meetings

and trainings.

o Assists program management and other staff in preparation of routine reports and in the completion

of various program related administrative tasks.

Participates in strategic planning, program improvement work groups, client/stakeholder satisfaction

surveys, peer review and other activities as requested by supervisor.

o Actively pursues development of professional competencies related to job role performance through

reading, supervision, in-service training and attendance at conferences and workshops.

o Awareness and active support of the Agency's Mission, Vision, Values and Strategic Plan. Including,

but not limited to, supporting the Agency's Core Competency initiatives of being Collaborative and

Community Focused, Innovative and Entrepreneurial, and Culturally Competent and Diverse

o Other duties as assigned.

Qualifications

Education: High School Diploma or Associate’s degree. Bachelor’s Degree in Social Work, Human Services,

or related field would be preferred. Equivalent combination of education and experience will be

considered.

Credentials: Valid and clean NYS Driver’s License and use of a registered and reliable vehicle.

Experience: At least one year of experience in the Human Services field and/or case management experience.

Experience working with volunteers, refugees, and local service providers preferred. Ability to

speak English is required. The ability to speak other languages is also preferred.
