Part time Case Manager
About the Role: Part time position with potential for Full time in the future! In-Home community-based position covering our West Region and surrounding areas: Springfield, Ware, Chicopee, Northampton, East Hampton, Holyoke, West Springfield, Westfield, Agawam, Huntington, and Deerfield.

Job Purpose
To develop positive relationships with providers, healthcare organizations and other community services
To coordinate the provision of social services to all program participants
To evaluate all clients in the program, and to plan and provide client, nurse and healthcare providers with appropriate plan of care recommendations
To maintain all client charts in a neat, organized fashion
To complete all required client paperwork in accordance with company procedures and state law
Essential Functions
Conduct on-site visits with each client and caregiver at the qualified setting
Obtains a social history and conducting a psycho-social evaluation
Conducts the initial and ongoing evaluations of the AFC and/or GAFC setting
Select, train, evaluate, and supervise AFC caregivers in conjunction with the registered nurse
Participate in the development, implementation, and ongoing review of the AFC plan of care
Presents, plans, and implements social service interventions
Complete a care-management progress note corresponding with each on-site visit or encounter and upon significant change
Assist with obtaining information and accessing other healthcare and community services
Participate in discharge and transition planning and implementation
Review and documenting the fire and safety procedures for the qualified setting
Review AFC caregiver task logs at a minimum of every 30 days
Review at least annually the suitability of the qualified setting.
Provide clients with ongoing support with ADL's and IADL's
Provide accurate orientation to the participants of the AFC and/or GAFC program
Coordinate all social services with the personal care team
Partake in both care plan creations and reviews, along with discharge plans
Has direct contact with the client and the client's caregiver and nurse
Takes care of all paperwork associated with Case Manager for admissions, referrals, visits, and medical equipment/social referrals
Assist participants with finding community, financial and legal services
Review and assess caregivers clothing, equipment, supplies and other services
Performs any other additional duties that may be required
Participates in weekly clinical meetings in the Braintree office
Conducts weekly calls to AFC Caregivers to confirm compliance with program regulations
Other duties as assigned
Physical and Environmental Demands:
- May be subjected to intense heat or cold external temperatures while traveling to client's homes. Requires substantial periods of repetitive work utilizing a touch screen computer, keyboard and mouse. Requires lifting and carrying equipment and supplies weighing up to 35 pounds; requires pushing and pulling equipment and supplies weighing up to 35 pounds; requires walking, standing, stooping and bending; requires frequent sitting; requires the ability to negotiate stairs; requires frequent talking, visual and hearing acuity, depth perception, and manual dexterity to operate equipment.
Education, Experience and Licensure Requirements
Bachelor's degree in human services, public health or a related field required
Valid MA Driver's License and reliable transportation required
Fluency in both English & Spanish language Strongly Preferred
Proven working experience in case management or human services
Excellent knowledge of case management principles, healthcare management, and reimbursement
Previous experience with mental health aspects of care
Strong communication, writing, organizational, and time management skills
Solution-oriented with the ability to multitask
Proficiency with the Microsoft Office Suite
