Responsibilities

This is a fulltime on-site position and may be located in our On-Site Virtual Care Center in Phoenix, AZ or may sit at our Rancho Cordova, CA National office location.

Position Summary

This position is an integral member of the TeleTracking Patient Logistics (PL) multi-disciplinary process team designed to develop patient flow best practice guidelines, define measures of patient care through the development and reporting of clinical quality indicators, and optimize efficient delivery of care via use of the TeleTracking® suite of applications and products. Serves as a champion for process change activities for Dignity Health PL. Helps define, along with the PL Managers and Directors, the direction for all efforts for department initiatives. Leads the formulation of the plans via value stream analysis along with appropriate follow-up actions. Identifies organizational and cross-organizational learning needs related to new/updated processes; coordinates resources and facilitates training activities to change the internal culture of the organizations involved. Manages the internal assessment process to determine progress along the journey and establishes an effective sustainment model. Acts as a catalyst for change and motivational force. With the support of PL leadership, drives communication and internal marketing efforts regarding the need for change. Coordinates and manages relationships with leaders and line staff in partner organizations, designated consultants and corporate colleagues. Creates and maintains the infrastructure to support PL as a management/leadership system integrating it into the unique culture of partnering organizations.

Accountabilities :

Planning and Managing Performance Expectations

1.1 Serves as advisor for setting strategic goals and measures for progress.

1.2 Assists with implementation of the short and long-term roadmaps for the partnership using the Teletracking Patient Throughput Analysis process.

1.3 Identifies upcoming events with appropriate personnel.

1.4 Coordinates all activities with staff and senior management.

1.5 Identifies and removes roadblocks and barriers that hinder the progress of the effort.

1.6 Coordinate execution of the strategic plan with the GSSA PL leaders.

1.7 Continually researches best practice and industry standards to stay abreast of cutting edge changes/updates.

Tracking and Reporting Performance Expectations

2.1 Develops and monitors tracking efforts from past and current initiatives and translates into detailed project plans.

2.2 Develops and tracks any corporate forms that measure and communicate progress.

2.3 Monitors Visual Dashboards and Performance Scorecards.

2.4 Monitors open issues from all activities, groups and committees.

2.5 Reports performance to Patient Logistics leaders and Steering Committee at least monthly.

Communicating and Training Performance Expectations

3.1 Serves as advisor to management and hospital site leads.

3.2 Coordinates with Patient Logistics leadership monthly Steering Committee meetings.

3.3 Serves as point of contact for consultants.

3.4 Contributes to development and execution of communication and training plans.

3.5 Coordinates training and staff on-boarding as needed.

3.6 Coordinates with other locations (best practices, methods, tools, etc.).

3.7 Identifies issues and provides solution recommendations.

3.8 Uses Lean tools and techniques to drive improvements in overall supply chain and quality performance.

3.9 Collaborates and communicates with Dignity Health quality members.

Marketing Performance Expectations

4.1 Communicates initiatives to internal and external parties as appropriate.

4.2 Communicates local issues/successes to internal audiences.

4.3 Manages best practice sharing and cross participation with appropriate facilities.

4.4 Provides constant visibility of the Patient Logistics projects in progress.

Qualifications

Experience : Five years experience in hospital operations including at least one year in a patient flow related role. TeleTracking system support experience preferred

Education : Bachelor’s Degree in healthcare related field or equivalent experience in Business Administration, Health Care Administration, Operations Research or other related field

Training : Prior training in Teletracking preferred.

Special Skills Required : Superior project management skills with an emphasis on interpersonal communication. Ability to lead in a team-oriented environment. Excellent organizational skills required, with the ability to manage multiple projects simultaneously. Able to operate within all levels of the organization. Ability to prioritize, plan and execute. Excellent oral and written communication skills. Proficient at Microsoft Office Suite: Word, Excel and PowerPoint and Google business applications. Passion for energizing the organization. Must be able to navigate and be proficient in hospital and business applications as a key requirement for this role. Ability to mitigate risk; change agent and leader.

Interpersonal Relationships: Must have excellent interpersonal skills to effectively build relationships within and outside the organization. Has regular contact with a variety of audiences including front line staff, management/senior management, physicians and other Dignity Health Divisions and Corporate staff. Has periodic contact with representatives of outside agencies including legal counsel, regulatory agencies, consulting services, other hospitals, etc. Must possess effective mitigation and conflict resolution skills.

Reporting Relationships : Reports to the Director of Patient Logistics

Travel : As needed

Pay Range

$34.50 - $40.27 /hour
