POSITION SUMMARY

Northern Rivers Family of Services is seeking a Program Assistant who will maintain smooth and patient-centered care for clients receiving services from the Assertive Community Treatment (ACT) program from intake to discharge.

PROGRAM ASSISTANT – Assertive Community Treatment (ACT)

The ACT Program Assistant is responsible for maintaining smooth and patient-centered care for clients receiving services from the primary licensed Assertive Community Treatment (ACT) program from the referral and intake process through ongoing treatment and discharge.

    Essential Duties and Responsibilities of the ACT Program Assistant

  • Maintain information of contact information for local first responders and community health agencies.

  • Develop and track a staff safety and monitoring system to include staff check-ins, and respond accordingly in case of staff or client incident.

  • Alert community partners and referral sources when there are openings in the specialized program.

  • Maintain accounting and budget records for recipient and program expenditures.

  • Develop and maintain proficiency in using various agency software programs.

  • Manage the referral, intake, treatment, and discharge process for the ACT program on a daily basis which includes coordinating appointments and processes with other program staff.

  • Work with Health Home to assist in shared information between Health Home and ACT team.

  • Management of program staff follow up calls to the discharged participants.

  • Compile meeting minutes, general as well as specialized program data and various reports for ACT program stakeholders.

  • Responsible for assisting in the development of program budgets, recording expenses, and tracking program spending.

  • Schedule Psychiatrist/Psychiatric Nurse Practitioner appointments as directed by Team Lead and ensure that all appointments are correctly scheduled for program and alert clinicians and prescribers when various documentation is required.

  • Other duties as assigned.

    EDUCATION and/or EXPERIENCE

  • Requires an Associate’s degree and three years of related experience, or requisite education and experience, Bachelor’s degree preferred.
