The RN Care Delivery Manager is responsible for supporting nursing standards of excellence through advanced professional education to deliver high-quality care. This role involves serving as a member of the management team, collaborating with all departments and employees to oversee day-to-day operations. The RN Care Delivery Manager provides guidance and implements best practices to achieve the organization's objectives, goals, and mission. The role is dedicated to improving the experience, satisfaction, and retention of patients, providing care coordination that is sensitive to diverse backgrounds, including age, gender, sexual orientation, cultural, linguistic, racial, ethnic, religious backgrounds, and disabilities.

Responsibilities

  • Support the nursing standards of excellence through advanced professional education.

  • Serve as a member of the management team, collaborating with all departments and employees.

  • Oversee day-to-day operations to ensure high-quality care delivery.

  • Provide guidance and implement best practices to achieve organizational objectives and mission.

  • Cultivate an environment dedicated to improving patient experience, satisfaction, and retention.

  • Coordinate care in a manner sensitive to diverse backgrounds and needs.

Essential Skills

  • Current active and unrestricted license and registration in New York State as a Registered Nurse.

  • Minimum of two years of administrative experience in a management capacity in a certified home health agency, long-term home health care, acute care, medical-surgical, and/or critical care, nursing home experience, diagnostic & treatment clinic preferred.

  • Customer service experience is required.

  • Managed long-term care insurance experience is beneficial.

  • Minimum of one year of experience working with a frail or elderly population, or appropriate training from the PACE organization upon hiring.

  • Supervisory experience is required.

Additional Skills & Qualifications

  • Be legally authorized to practice in the State in which the healthcare professional will perform the function.

  • Be medically cleared for communicable diseases and have all immunizations up-to-date before engaging in direct participant contact.

  • Bilingual preferred.

  • Care management and case management experience.

Work Environment

The role is based in an onsite medical clinic within a community healthcare setting. The work hours are from 8:30 AM to 5:00 PM, totaling 37.5 weekly hours. The work environment is supportive and collaborative, focusing on delivering high-quality patient care.
