General Statement of Duties

The Population Health Specialist is responsible for aligning the population health strategies and interventions set by the Population Health Partnerships department. Key functions of this position include providing project management, contract monitoring, report writing, process improvement, and coordination and facilitation of stakeholder meetings in support of improving the health of our population. This position will spend the majority of time supporting the Population Health Team and clinical and community stakeholders in order to meet annual clinical population health strategies.

Essential Position Functions

  • Provide ongoing support to CareOregon’s CCO Population Health Portfolio Managers (PHPMs) in the implementation of population health initiatives for population level change.

  • Assist PHPMs in the communication and translation of population health strategies, priorities, best practices and updates, as well as internal CareOregon team support.

  • Act as a liaison between internal and external partners, contractors and stakeholders to facilitate collaboration and consensus building; help ensure accountability, as well as maintain technical project reports, records and documentation.

  • Implement project plans that align with the objectives of the Population Health Partnership department and CCO leaders and report back and coordinate deliverables with stakeholders.

  • Attend and participate in population health meetings and provide project management or staff support, as needed, and in collaboration with other internal CareOregon CCO team members supporting population health work.

  • Work with leadership and internal CareOregon teams to ensure that all stakeholders receive status reports they need to be adequately apprised of progress, to make decisions, and to move their deliverables forward; assist with reviewing data and information in order to identify opportunities for programmatic improvement.

  • Work with the Population Health team and stakeholders to shape population health portfolio of work to best meet their needs; identify barriers to meeting key population health priorities through the establishment and facilitation of leadership meetings with the CCO and other stakeholders.

  • Apply appropriate improvement tools to meeting population health goals and communicate with other departments and stakeholders as necessary.

  • Participate in appropriate statewide learning collaboratives related to population health projects.

  • Participate with leadership in creating and implementing evaluation tools for key process and outcome measures for population health interventions and contracts with the network.

  • Provide workgroup/collaborative support as needed.

    Knowledge, Skills and Abilities Required

  • Knowledge of population health, behavioral health and oral health

  • Understanding of project management fundamentals; ability to develop a formal and detailed project plan aligned with established timeline and budgetary guidelines

  • Effective ability to lead and motivate peers, cross-functional partners and management toward project goal attainment

  • Ability to identify and anticipate issues and act proactively to formulate plans and recommendations

  • Strong ability to manage multiple activities, matrixed teams, organization communications, and frequently competing priorities to implement change as necessary within time and budget constraints

  • Skill in lean and or other quality or process improvement

  • Advanced computer application skills including with MS Windows, Word, Excel, and Outlook with proficiency using Power Point, Access, Visio and internet browsers/search functions

  • Ability to articulate and advocate for CCO strategies and priorities with community partners and with internal CareOregon staff

  • Strong communication skills, including listening, verbal, written, documentation, meeting facilitation, and presentations

  • Proficiency in conveying appropriate level of detail effectively to all levels of the organization

  • Ability to simplify and present complex concepts in an easily understood way

  • Ability to work in a community (internal and external) with diverse individuals and groups to build effective, maintain cooperative, and productive working relationships with department management team, internal and external customers

  • Ability to work independently as well as functioning in a positive team environment

  • Knowledge in critical analysis, quantitative reasoning, and creative problem-solving skills

  • Ability to function within tight time constraints

  • Ability to present a positive and professional image in all settings

    Education or Experience

    Required:

  • Minimum 3 years’ experience in clinical improvement, clinic administration, project management, or stakeholder collaboration work

    Preferred:

  • Experience leading complex improvement projects with multiple stakeholders

  • Training in lean or related process improvement techniques
