Job Summary

The Population Health Portfolio Manager is responsible for the oversight and continuous improvement of a regional Population Health Portfolio and initiatives. The position acts as an innovator and integrator across the network, convening Coordinated Care Organization (CCO) partners to co-create, learn together, and spread emerging best practices for improving care for CareOregon populations. This highly creative leadership position is responsible for promoting a great degree of intra-departmental collaboration and integration. Functionally, this position oversees the regional population health and high-risk strategy.

Essential Responsibilities

  • Participate in the development, implementation, and evaluation of population health programs, including high risk CareOregon members in partnership with the CareOregon network and other community partners.

  • Create, recommend, facilitate, and evaluate population health strategies and care enhancing cost-control programs that serve members across CareOregon’s regional population.

  • Serve as the content expert for Population Segmentation and other population health tools for the region.

  • Promote collaboration with delivery system leaders across the region to build a strong foundation of population health program support.

  • Develop and maintain strategic partnerships with community-based social service organizations in partnership with community health staff to assure CareOregon can leverage and improve access to upstream non-medical health-enhancing services for its members.

  • Support collaborative relationships with providers that promote and achieve Triple Aim goals.

  • Ensure strategic performance goals, expectations and standards are clearly understood by stakeholders.

  • Evaluate the Population Health Portfolio performance on an ongoing basis and take appropriate corrective action.

  • Represent CareOregon on appropriate local, state, and national committees and work groups.

  • Strategically inform and provide guidance from a population health perspective to care management teams.

    Employee Supervision

  • Manages team and recommends team direction and goals in alignment with the organizational mission, vision, and values.

  • Identifies work and staffing needs to meet work expectations; recruits and hires, using an equity, diversity, and inclusion lens.

  • Plans, organizes, schedules, and monitors work; ensures employees have information and resources to meet job expectations.

  • Leads the development, communication, and oversight of team and individual goals; ensures goals, expectations, and standards are clearly understood by staff.

  • Trains, supervises, motivates, and coaches employees; provides support toward employee development.

  • Incorporates guidance from CareOregon equity tools into people leadership, planning, operations, evaluation, and decision making.

  • Ensures team adheres to department and organizational standards, policies, and procedures.

  • Evaluates employee performance and provides regular feedback to support success; recognizes strong performance and addresses performance gaps and accountability (corrective action).

  • Performs supervisory tasks in collaboration with Human Resources as needed.

    Experience and/or Education

    Required

  • Minimum 5 years’ related experience, including 3 years in a population health program

  • Minimum 2 years’ experience in a supervisory position or minimum 1 year experience in a supervisory position with completion of CareOregon’s Aspiring Leaders Program

  • Valid driver’s license, acceptable driving record, and automobile liability coverage or access to an insured vehicle

    Preferred

  • Leadership experience in a managed care organization

    Knowledge, Skills and Abilities Required

    Knowledge

  • Strong understanding of impact of generational poverty and other social determinants of health on clients’ ability to develop and follow self-care goals to improve health outcomes

  • Applied knowledge of program management and ability to lead and/or contribute to process improvement strategies

  • General understanding of the management of diverse medical and psychosocial problems, and a deep understanding of the health effects related to poverty, societal discrimination, and intergenerational trauma

  • Understanding of managed care and publicly financed health care stewardship principles

  • Understanding of health equity, health literacy, shared decision making, and advanced care planning principles

    Skills and Abilities

  • Ability to design and implement new initiatives and strategies that improve population outcomes

  • Strong people leadership skills, including the ability to lead a diverse group of people toward a common vision with proven results related to motivation, engagement, and goal attainment

  • People management skills, including the ability to coach and develop staff

  • Highly effective strategic planning, communication, organization, continuous improvement, and interpersonal skills

  • Excellent written and verbal communication skills

  • Ability to train diverse groups about population health, complex care management, and triple aim principles

  • Ability to work with an open leadership style and actively seek out and support collaborative thinking and problem solving with others inside and outside the organization and approaches his/her work as an interconnected system

  • Ability to learn, focus, understand, and evaluate information and determine appropriate actions

  • Ability to accept direction and feedback, as well as tolerate and manage stress

  • Ability to see, read, and perform repetitive finger and wrist movement for at least 6 hours/day

  • Ability to hear and speak clearly for at least 3-6 hours/day

  • Ability to operate a motor vehicle
