Family Medical Group, NE , part of Optum, has an immediate opening for a friendly, patient focused and detailed oriented Patient Care Coordinator to join our team. Patient Care Coordinator is responsible for the completion of set processes and protocols. Works cooperatively with all members of the care team to support the vision and mission of the organization, deliver excellent customer service and adhere to Lean processes. Supports the teams in meeting financial, clinical and service goals.

Schedule : The hours are Monday through Friday a 40 hour work week to be determined by the supervisor between the hours of 7:00 am to 5:00pm. Rotating Saturdays between the hours of 8:00 am to 1:00 pm.

Primary Responsibilities:

  • Applies knowledge/skills to activities that often vary from day to day

  • Demonstrates a moderate level of knowledge and skills in own function

  • Requires little assistance with standard and non-standard requests

  • Solves routine problems on own

  • Works with supervisor to solve more complex problems

  • Prioritizes and organizes own work to meet agreed upon deadlines

  • Works with others as part of a team

  • Assist physicians, as required in coordination of care for patients

  • Works collaboratively with Centralized Services to support patient needs

  • Establish and maintain relationships with service providers for patient’s coordination of care

  • Ensures work is being performed according to company policies, procedures, and standards

  • Provides high quality care and service excellence to our patients

  • Performs other administrative and clerical duties as assigned

  • Participates in a variety of department initiatives

  • Ensures all OSHA, HIPAA, and other regulations are followed

  • Follows proper reporting protocols for any compliance or incident reporting as needed

  • Maintains strict patient and employee confidentiality

  • Other duties and responsibilities, not specifically described, maybe defined, or assigned from time to time, consistent with knowledge, skills and abilities of the incumbent by management

Required Qualifications:

  • High School Diploma/GED (or higher)

  • 1+ year(s) of Microsoft Office experience

  • 1+ year(s) of customer service experience

Preferred Qualifications:

  • Experience with data analysis software

  • 1+ years of electronic medical record experience

  • 1+ years of health plan insurance knowledge

  • Bilingual (English / Spanish)
