Provider Solutions & Development Nurse Practitioner or Physician Assistant Palliative Care in Portland, Oregon

Providence Connections Palliative Care is seeking a board-certified Nurse Practitioner to join a well-established, trans-disciplinary team providing palliative care to patients and their families in primarily outpatient setting. This role will provide evaluation and management of chronic conditions or serious, life-limiting illness. The ideal candidate will have previous palliative, oncology, and/or geriatric care experience. Connections Palliative Care is part of a growing integrated Home and Community Services program which collaborates with Hospice, Home Health, Geriatrics as well as hospital-based practitioners to care for patients and their families.

  • Part-time at 0.9 FTE (considered full-time for benefit purposes)

  • Monday - Friday work week, daytime hours

  • Collaborate with physicians, nurses, social workers, and chaplains

  • Minimum of one year of post-graduate clinical experience in palliative care, hospice, geriatrics, and/or oncology care is preferred

Where You’ll Work

Providence Connections is a Palliative Care consultation service for patients living with a serious illness. This interdisciplinary group helps patients and their families find needed resources to control symptoms associated with their illness and maximize quality of life, all while providing compassionate support. Teams of physicians, nurse practitioners, Palliative Care nurses, clinical social workers and chaplains work collaboratively with all members of patients’ care teams to provide holistic care.
