Maxim Healthcare Services Transition Team

Our Transition Team is a supplemental care unit of skilled nurses whose primary function is to quickly and efficiently get cases home while primary nursing staff are actively recruited. The team also supports the local office by covering potential missed opportunities.

How it works: 

Once a referral is received, we immediately begin sourcing full-time staff. Prior to discharge from the facility, patients will meet an internal office representative to detail how the program works. Once the patient transitions home, we'll employ our nurses to ensure full coverage until the permanent nursing staff can be identified.

Transition team nurses provide care for all authorized hours in a home during the provisional period of 1-120 days. During this time, out office staff will be recruiting to identify full-time, replacement home healthcare nurses. The new caregivers will have an opportunity to train and shadow in the home with the Transition team caregivers prior to working independently with the patient. Once the replacement nurses are trained and have a schedule, the transition team are then phased out of the home.

  • If there is an occasional fill-in need for a long-term caregiver, our Transition Team will step in to cover any gap shifts (if available)

Transition Team Requirements

  • Minimum one year of nursing experience
  • Must be approved for TT by internal RN during office interview
  • Must pass office competency/skills validation on the following items:
    • Trach
    • Vent
    • G-Tube
    • Proper Documentation
  • Ideal candidate will display above average customer service, communication, and professionalism qualities.
  • Candidate must display understanding of Maxim's emphasis/requirements of the following items:
    • Consistent communication with internal staff
    • Ongoing vigilance with securing PHI, meeting all HIPPA regulations, and security of patient and staff privacy.
    • Ability to maintain a schedule that will change frequently
    • Possibility of lengthy commute times to and from work
    • Fortitude to work in varying home environments
    • Must be able to work a minimum of 4 weekend shifts a month

Transition Team Pay Scale

  • RN: $36
  • LPN: $34
