The Bradley Center Case Manager in Pittsburgh, Pennsylvania
Since 1905, The Bradley Center has continually served children and families in need. Today, the residential treatment facility provides 24 hour care and trauma-informed treatment to children who have a history of trauma and who may present with a wide range of challenges.
We are seeking a Case Manager to provide quality therapeutic care management services for children and their families as part of a residential treatment team.
Primary responsiblitlies include:
Schedule and facilitate Interagency team meetings
Coordinate treatment services and discharge planning
Conduct chart audits
Communicate with families and external sources effectively and in a professional demeanor
Complete required documentation of tasks completed, complete monthly status updates and treatment plan updates
Requirements
Bachelor’s degree in social work, counseling or other related field
Minimum of one year experience working in a human services field (CASSP system-related field preferred)
Must be at least 21 years of age
Valid driver’s license and car insurance
