Our 70-bed facility provides both short-term rehabilitation and long-term care for individuals requiring skilled nursing. As an MDS(RN) Coordinator you are responsible for coordinating the completion of the comprehensive and Medicare required assessments with the rest of the ID team. These assessments are the primary driving force behind resident holistic based care. This position will Implement care plans and review and update care plans quarterly. This is a full-time day position with shared on-call and holiday rotation responsibilities. Responsibilities should include: A current, unencumbered RN (Registered Nurse) license in the State of Minnesota. Must be familiar with the standards of nursing practice and state and federal regulations. 2 years of long-term care and MDS (Minimum Data Set) experience preferred Good oral and written communications skills.
