The RN Case Manager: provides quality case management and qualified professional support and services to inpatient social workers as assigned; works with local agency and service providers to facilitate sending and receiving collateral information, treatment referrals, and ongoing communications to support patient’s successful inpatient treatment and discharge.

Qualifications

The Nurse Case Manager is responsible for the assessment, planning, implementation, coordination, monitoring, and evaluation of the patient’s plan of care during admissions. The Nurse Case Manager utilizes clinical knowledge, critical thinking skills, and the principles of case management to bridge the admissions process for patients from the Crisis Response Center to an inpatient unit.

Job duties include reviewing direct admissions for medical needs and ensure criteria for admissions are met, performing assessments and assisting inpatient nurses when needed to complete the admissions process. The Nurse Case Manager will work collaboratively with interdisciplinary staff internal and external to the organization. Participates in quality improvement and evaluation processes related to the management of care.

Education/Qualifications

  • Registered Nurse, Associate's degree required (BSN preferred)

  • Case Management Certification preferred
