Penn Medicine is seeking a full-time, day shift, Clinical Pharmacy Specialist, in Hematology / Oncology at Penn Presbyterian Medical Center and Penn Presbyterian Infusion Services.

Summary:

Responsible for provision of pharmaceutical care with a focus on infusion chemotherapy to patients managed at the Penn Presbyterian Medical Center sites in Philadelphia and South NJ locations. The specialist will be expected to develop a practice within the hospital and various ambulatory practice sites, and act as a liaison with other partners.

Activities include evaluate, monitor and modify drug therapy, provide recommendations to optimize patient outcomes, provide personnel/patients with new medication education as needed, minimize safety issues and assure cost effectiveness as well as provide Drug Information for medical personnel.

The pharmacist should be an effective communicator and be able to integrate themselves within the health team and current structure of the pharmacy department. Additionally, the candidate should coordinate pharmacotherapy activities and educational opportunities for department pharmacists and residents associated with hematology/oncology therapies.

The pharmacist will also lead and assist with collection and presentation of patient outcomes data and quality improvement projects. Finally, the clinical pharmacy specialist should fulfill requirements associated with hospital and department policies and procedures, JCAHO and other regulatory agencies.

Responsibilities:

  • Provides optimal pharmacy service to patients and other health care professionals by designing, recommending, monitoring, evaluating and documenting patient and age-specific pharmacotherapy.

  • Participates in the education of all healthcare providers regarding safe and effective medication use. Precepts pharmacy students, residents and pharmacy staff as assigned. Provides pharmaceutical education to patients and caregivers as needed

  • Verifies, prepares and dispenses medications using appropriate techniques to ensure positive outcomes of drug therapies and compliance with regulatory requirements and the organization’s policies and procedures. Ensures adherence to medication-use policies and guidelines

  • Provides concise, applicable and timely responses to drug information requests from health care providers and patients

  • Participates in the management of medical emergencies

  • Contributes to, participates in, and leads multidisciplinary quality improvement initiatives, drug use evaluations, and clinical research within the pharmacy and/or other institutional departments to improve patient care

  • Optimize cost-savings of operations and lead cost-savings initiatives. Oversee and provide support in areas of insurance and billing information

  • Supervises and directs the activities and growth of other pharmacists, residents, technicians, interns, and students

  • Works to advance the practice of pharmacy through contributions to the medical and pharmaceutical literature as well as through participation in professional organizations

  • Manage medication safety issues and participates in and leads medication safety measures.

  • Optimize patient outcomes while ensuring cost effectiveness

  • Clinical Pharmacy Specialists are expected to be leaders in their respective areas to assure the highest quality and most up to date pharmaceutical care is provided to patients.

Accountabilities:

  • Evaluate, monitor and modify drug therapy

  • Provide patients and staff with new medication education as needed

  • Coordinate pharmacotherapy activities and educational opportunities for department pharmacists and residents in the inpatient and ambulatory practice setting

  • Assist with the collection and presentation of patient outcomes data

Credentials:

  • PA Pharmacist License (Required)

  • NJ Pharmacist License (Preferred)

  • Board of Pharmaceutical Specialties Certification (preferred) in area of specialty practice

Education or Equivalent Experience:

  • Bachelor of Arts or Science

  • Pharmacy- Bachelor of Science Degree or Doctor of Pharmacy Degree from an accredited College of Pharmacy Personal commitment to advancing the profession. Demonstration of contributions to pharmacy practice and leadership skills

  • Successful completion of a PGY1 Pharmacy Residency OR as a licensed pharmacist in a hospital pharmacy with >/=50% of time in area of specialty. Successful completion of a PGY2 Specialty Residency preferred. Preferably in Hematology/Oncology OR as a licensed pharmacist in a hospital pharmacy with >/= 50% of time spent in area of specialty.
