Job Description/Essential Functions:

Horizon House, Inc. is a nationally recognized provider of services to adults with support needs due to developmental disabilities, behavioral health issues, and substance abuse. Horizon House is Pennsylvania's oldest psychiatric rehabilitation agency. Since 1952, we have been dedicated to helping people with mental illness, drug and alcohol addictions, developmental/intellectual disabilities, and homelessness to stabilize their lives and to help them live as independently as possible in the community. With over 100 locations throughout South and North Eastern Pennsylvania and the state of Delaware, Horizon House provides services to a diverse population focusing on individual strength and choice.

Currently, we are seeking a Full-Time Case Manager to work in our Homeless Services Division. The position provides community-based supports to individuals in compliance with the Agency service contract and policies and procedures. Approximately 75% of work is done in the community. The selected candidate does home based visitations and interacts with members of the multidisciplinary team to ensure that participants receive individualized services designed to meet their needs and to help them successfully maintain housing and accomplish their recovery goals. Some of the responsibilities include the following: Providing engagement and case management support including home-based visitations and other services within the Philadelphia community. Approximately 75% of work is in the community.

Interacting with members of the multidisciplinary team to ensure that participants receive the individualized services designed to help them successfully maintain their permanent housing.

Providing services in compliance with contract and Agency regulations. Conducting and coordinating outreach activities designed to locate and engage TCMU authorized participants. Providing clinical services to participants as needed in their natural environment or in their program housing or assist participants to access mainstream clinical services. Working with participants to develop, implement and monitor their personal goal plans to enable them to stabilize and retain housing on a permanent basis.

Job Qualifications:

High School diploma or General Equivalency Degree (GED) and at least 2 years of direct care experience working with the target population; or any combination of experience, education or training that would provide the level of knowledge, skill and ability required.

Basic understanding of Mental Health, and/or Substance Abuse issues.

Very good communication skills.

Detail oriented.

Ability to work independently and as part of a focused team.

Scope:

Frequent talking and listening

Requires the ability to lift, carry, fold, kneel, reach, stack, stoop and bend which may include direct intervention with clients.

Must be available for local travel and possess a valid Driver's license.

Possess manual dexterity and fine motor skills.

May require crisis intervention to emergency situations.

Will be required to carry a mobile phone during regularly scheduled working hours.
