Job Description/Essential Functions:

The purpose of this position is to engage and support participants so they address their recovery needs and successfully maintain permanent housing.

  • To provide assistance and support to Team Leaders and Case Managers.

  • To provide services in compliance with contract and Agency regulations.

  • Approximately 85% of the work is in the community.

Job Qualifications:

This position requires a High School diploma or General Equivalency Degree (GED) and at least 2 years of direct care experience working with the target population; or any combination of experience, education or training that would provide the level of knowledge, skill and ability required.

  • Basic understanding of Mental Health, and/or Substance Abuse issues.

  • Very good communication skills.

  • Detail oriented.

  • The ability to work independently and as part of a focused team.

  • Frequent talking and listening

  • Requires the ability to lift, carry, fold, kneel, reach, stack, stoop and bend which may include direct intervention with clients.

  • Must be available for local travel and possess a valid Driver's license.

  • Possess manual dexterity and fine motor skills.

  • May require crisis intervention to emergency situations.

  • Will be required to carry a mobile phone during regularly scheduled working hours.

  • The ability to serve as an advocate and encourage clients to assume a proactive role in their rehabilitation process. Provide direct support through effective communications with program staff, clients and other providers.

  •  The ability to submit accurate documentation and related information, on a timely basis. Document all contacts, movement, growth, transfers and overall progression through the system, in accordance with funding source regulations.

  • The ability to provide services to participants as needed in their natural environment or in their program housing. Provide hands-on support in transporting clients to appointments, in shopping, and in learning skills of daily living.

  • Ability to provide outreach services to locate and engage service participants.

  • Ability to serve as an active participant in the team to provide comprehensive and flexible services to participants.
