Job Description/Essential Functions:

Horizon House, Inc. is a nationally recognized provider of services to adults with support needs due to developmental disabilities, behavioral health issues, and substance abuse. Horizon House is Pennsylvania's oldest psychiatric rehabilitation agency. Since 1952, we have been dedicated to helping people with mental illness, drug and alcohol addictions, developmental/intellectual disabilities, and homelessness to stabilize their lives and to help them live as independently as possible in the community. With over 100 locations throughout South and North Eastern Pennsylvania and the state of Delaware, Horizon House provides services to a diverse population focusing on individual strength and choice.

We are seeking qualified candidates for the position of Case Manager Aide. The position has responsibility to engage and support participants so they address their recovery needs and successfully maintain permanent housing.  Provide assistance and administrative support to the program's Team Leader, Mental Health Specialists and/or Case Managers. Provide services in compliance with contract and Agency policies and procedures. Other responsibilities include the following: *Actively participates as a member of a multidisciplinary team of professionals focused on the provision of comprehensive and flexible services to participants. Expected to contribute to a participant centered program culture that is outcome-oriented and committed to quality improvement. *Provide services to participants as needed in their natural environment or in their program housing. Provide hands-on support in transporting participants to appointments, in shopping, and in learning skills of daily living. *Submit accurate and timely documentation and related information in accordance to TCMU/CBH/HH standards. Document all contacts, movement, growth, transfers and overall progression through the system, in accordance with funding source regulations. Expected to consistently meet daily productivity standards as required by the funder and department. *Consistently meet daily productivity standards as required by the funder and department. *Conducts and coordinates outreach activities designed to locate and engage TCMU authorized participants.

Job Qualifications:

*High School diploma or General Equivalency Degree (GED) and at least 2 years of direct care experience working with the target population; or any combination of experience, education or training that would provide the level of knowledge, skill and ability required. *Skills and competence to establish supportive trusting relationships with persons with severe and persistent mental illnesses and respect for participant rights and personal preference in treatment are essential. *Ability to work independently and as part of a focused team. *Working knowledge of Outlook, MSWord and Excel. *Valid driver's license with an acceptable driving record required. *Ability to maintain daily productivity standards. *Fee-for-service and electronic health record experience strongly preferred.

Scope: *Frequent talking and listening. *Requires the ability to lift 25 pounds, carry, fold, kneel, reach, stack, stoop and bend which may include direct intervention with participants. *Possess manual dexterity and fine motor skills. *Must be available for local travel and. *Must carry a mobile phone during scheduled working hours. *As essential personnel, may be required to report to work during emergencies including inclement weather.
