Community Wellness Partners Per Diem Occupational Therapist in Oswego, New York

$55.00 - $60.00/hour - Based on Experience

We are growing! St. Luke Health Services in Oswego is seeking a per diem Occupational Therapist (OT) to be part of its “CAPABLE” Team. The CAPABLE program is a best practice model developed by Johns Hopkins University School of Nursing. It is a free innovative program with the focus of maintaining a safe home environment for older adults. OT will work with older adults to deliver the in-home occupational therapy components of the CAPABLE program. The in-home services are participant driven and are a collaboration of nursing, OT and handy man.

Essential Job Functions

  • Completion of required training through Johns Hopkins University School of Nursing

  • Follow the structure and outline of the CAPABLE program.

  • Communicate and coordinate with the CAPABLE Registered Nurse and Program Coordinator to ensure proper implementation of CAPABLE program.

  • Observe the participant’s functional ability within their home to determine barriers

  • The OT will work with and encourage CAPABLE participants to identify both behavioral and environmental obstacles to maintaining a safe home environment.

  • Engage participants to problem solve and develop action plans to address the identified safety, behavioral and environmental barriers to a safe living environment.

  • Communicate and coordinate with handy workers and landlords to develop prioritized work orders for home repairs and modifications to assist in meeting the participant goals.

  • Identify and assist in purchasing necessary items to assist in meeting the participant goals.

  • Re-evaluate, Review, Reinforce and strategies, problem solving for current and future needs of the participant.

  • Maintain and manage equipment used by participants (inventory, order, transport) and direct/monitor handy worker activities according to the work orders.

  • Address concerns and issues the participants have as they arise.

  • Maintain necessary documentation and data.

    WORK ENVIRONMENT:

    Ability to work from a collaborative, client-centered model. Willingness and ability to implement a protocol and balance with clinical judgment. Work is performed in the participant’s home setting and in an office setting. Requires walking up to 95% of the workday. Must be capable of pushing, pulling, and moving up to 50 pounds or more.

    QUALITIFICATIONS:

    Degree and New York State licensure to practice occupational therapy.

    Valid NYS Drivers license and reliable transportation

    Effective Communication and assessment skills

    Ability to engage and empower participants

    Active listening skills

    Computer literate

    Innovative Problem Solving

    Preferred Qualifications: Experience with older adults highly preferred.
