Community Based Care of Brevard, Inc. Behavioral Health Coordinator - Post Adoption in Orange/Osceola County, Florida

Minimum Starting Salary: $62,000/annually Position Summary: The Behavioral Health Coordinator (BHC)-Post Adoption is responsible for c oordination of behavioral health services for adopted children enrolled in the Child Welfare Specialty Health Plan through Sunshine Healthcare. This position coordinates community mental health, clinical and substance abuse treatment services and discharge planning for any child hospitalized or admitted to an inpatient facility. The BHC-Post Adoption is responsible for ensuring follow up appointments 7-day post discharge, documentation, and response to Sunshine Health the next business day after care coordination activities is performed. This position participates in MDT, Treatment Team Meetings and ad hoc meetings for any child requiring a higher level of care. Technical/Functional Expectations: Assessment and Care Coordination: Assists adopted children with clinical needs and provides care coordination of behavioral health services. Educates caregivers regarding medications, routine medical care, how and when to contact the child’s Primary Care Physician. Service Referral and Identification: Identified and secures services and compiles documentation to ensure service delivery, including service authorizations. Ensures consistent and ongoing collaboration with Sunshine Health with contacting the applicable caregiver and/or enrollee. Multi-Disciplinary Team (MDT) and Treatment Team Meetings: Provides coordination and facilitation of Multi-Disciplinary Team (MDT), Treatment Team and other ad hoc meeting to review behavioral health needs including access to therapeutic placements and/or higher levels of care. Follow Up After Hospitalization after Mental Illness and Substance Use-Discharge Planning: Participates in discharge planning activities, following children’s admission into an inpatient hospital or crisis stabilization unit, to coordinate services and to ensure a seven (7) day follow up appointment. HEDIS Care Gap Reports: Review of health and wellness reports (i.e., HEDIS Care Gap Reports), provided by Sunshine Health, which indicate services and performance measures that are due, or past due. Provide follow-up and outreach as needed. Quality Management: Review case management reports/data, available within the Sunshine Health Secure Provider Portal (SPP) and will communicate with Sunshine team as required. Documentation: Ensure all care coordination activities are documented in compliance with Sunshine requirements. On Call Rotation: Participate in placement on call rotation to provide timely service. Other duties as required: Job performance requires fulfilling other incidental or related duties as assigned, assisting, and training others, and performing duties of higher rated positions from time to time for developmental purposes. Business Experience: Five years Clinical Experience: This is required to meet the unique behavioral health needs of children being served in Florida’s child welfare system of care. Strength Based Family Centered Practice Orientation: This is essential to effectively support the System of Care and its values. Strong Interpersonal, Presentation, and Organization skills: This is required to effectively partner with various stakeholders and agency staff and adequately train community stakeholders. 3-5 years Child Welfare Experience: This is preferred to effectively understand that improving the well-being of children in the child welfare system is a community effort. Valid Driver's License - Essential: This is required to travel throughout the county and other locations. Educational and Experience Requirements: Masters in social work or MA/MS in Human Services field required. Licensed or Licensure eligible preferred. 3 years of experience in child welfare preferred.
