Provides skilled home nursing per physicians orders.

Assesses patients needs and prepares patient care plan. Updates the care plan as changes occur.

Performs treatments and administers medications as prescribed by the physician in accordance w/ Oklahoma State Nurse Practice Act.

Assesses effectiveness of treatments and monitors the patients response.

Maintains complete and accurate clinical records on all patients assigned to his/her care.

Reports significant findings or changes in the patients conditions to the appropriate individuals.

Maintains credentials pursuant to the Oklahoma Board of Nursing.

Submits completed Visit Records every Monday for payment.

Coordinates with other services to provide a continuity of care.

Maintains compliance with applicable laws and regulations.
