Job Summary and Qualifications

*Shift: Monday-Friday (8:00AM-4:30PM), rotating weekends.

*This is a salaried position.

This position is accountable to the Program Director of the rehabilitation program. This position acts as a inpatient rehabilitation expert between internal and external referral sources, and inpatient rehabilitation operations. The Clinical Rehabilitation Specialist is responsible for establishing relationships with case managers, physicians, payors, and other referral sources. The Clinical Rehabilitation Specialist will ensure appropriate patient placement through clinical assessments and screening in collaboration with the Rehab Medical Director. The Clinical Rehabilitation Specialist assists the Program Director to develop and implement a referral development program as a means to promote the features and benefits of the rehab program to outside referral sources, CMs, patients, families and the community. The amount of time spent performing either task will vary based on volume of activity.

Licensure, Certifications, & Training:

  • Registered Nurse - Currently licensed as a registered professional nurse in the state in which he or she practices, in accordance with law and regulation. (Compact RN - When a nurse changes primary state of residence by moving from one compact state to another, the nurse can practice in the new state on the former license for up to 90 days. The 90 days starts when the nurse becomes a resident in the new state.

OR

  • Physical Therapist license, Occupational Therapist license, or Speech Language Pathologist license. Required.

Education & Experience:

  • Bachelors degree

  • Masters degree or higher

  • 1-5 years of experience

  • Ability to navigate a medical record for determination of medical necessity for an IRF admission

  • Ability to build strong relationships with referral sources while being engaging , accountable and responsive

  • Ability to analyze clinical data looking at diagnosis and comorbidities that will demonstrate the patients’ complete medical picture.

  • Experienced in speaking in medical terminology with physicians, case managers, therapists about disease statesx comorbidities and patient care.
