Job Overview:

The HIM Tech II will report to the Director, Ambulatory HIM. The Medical Records Tech II will primarily handle one of the following

  • Facilitate the processing of medical group requests for protected health information/medical records by reviewing the requests, entering the requests into the designated release of information (ROI) system and acts as a liaison with medical group practice staff and RWJBH system ROI vendor on questions regarding the status of requests.

  • Monitor document indexing application to keep all HIM workbaskets up to date. Will also interact with team from application vendor regarding questions. And, will prepare reports for distribution to practices regarding errors needing resolution.

The Tech II will also handle other duties related to Chart Completion and Document Corrections as needed.

Qualifications:

Required:

  • High School Diploma or GED required

  • Ability to handle a high volume of work

  • Ability to perform team-oriented job tasks.

  • Conscientious awareness to detail and accuracy

  • Ability to work independently

  • Strong customer service skills

  • Excellent communication skills (oral and written).

  • Experience with an electronic medical record

  • Ability to learn, understand and apply federal and state regulations.

  • Problem solving and critical thinking skills

  • Demonstrates a clear understanding of the importance of patient privacy

  • Individual must be able to meet established physical requirements of the position including sitting at a desk or meeting table, standing or walking for an extended period of time

  • Able to work on a computer for a full work day

  • Able to work remotely, in a completely electronic environment

Preferred:

  • 1-2 years prior experience in HIM in an ambulatory environment preferred

  • Associates degree and/or RHIT credential preferred.

  • Prior experience with Epic is preferred

Essential Job Functions

  • Identification of CJR, MSSP, HRRP, and other patients attributed to a value-based contract upon admission:

  • CJR patients are identified through the Ortho Service Line daily via an Excel list that is provided to the Episode Coordinator via a Shared Drive (patient tracking tool)

  • MSSP and other value-based patients (Non/CJR patients) are identified through the Acute Admit program in Epic

  • HRRP lives are identified through the HRRP program in Epic

  • Documentation of patient data and information in Epic Healthy Planet for CJR, MSSP, HRRP, and all value-based contracts patients. Specific documentation focuses on care at the time of discharge within Epic: what happened, where they went, both level and name of the facility, if applicable

  • Engagement with the Care Management Director and Physician Advisors

  • Identify units with the highest volume of VPC patients and discuss care planning, in real-time, with individual patients on the daily census

  • Daily touch base with Unit Case Management to facilitate appropriate following site of care

  • Actively communicates with discharge planners about patients with Post-Acute Care facility needs

  • Communication with Post-Acute Coordinators on patients going to facilities

  • Communication to Population Health Nurse care manager and VNA on patients going home with or without Home Health

  • Receive a risk stratification tool and Value-Based Care criteria to focus and prioritize efforts.

  • Conduct follow-up phone calls after the patient has been discharged from an acute care setting to home to screen patients for safe discharge and escalate any clinical issues as needed for clinical interventions.

  • Reports directly to the centralized value-based operations care management team, ensuring strong alignment and continuity of care for patients and avoiding any overlapping outreach.

  • Inform Case Management of readmissions

Essential Job Functions Cont.

  • The HIMTech II handles all requests and inquiries for PHI received through medical group practices.

  • The HIMTech II monitors/manages the RWJBH medical group medical records email for all ROI requests.

  • The HIMTech II will be responsible for daily entry of required information related to the PHI request into the designated ROI system.

  • The HIMTech II enters audits with multiple patient names per ROI system procedure. (Defined by procedure per 3rdparty vendor).

  • Requests for PHI related to Radiology films (RWJPE) will be processed by the The HIMTech II in collaboration with our ROI vendor

  • Requests for patient billing records (RWJPE) will be processed by the The HIMTech II in collaboration with our billing department.

  • Answers clerical telephone calls from practices and requestors pertaining to release of information

  • The HIMTech II will refer subpoenas, court orders, and other court related requests to medical group Privacy/Risk leadership for review.

  • Work with practice managers, staff, other members of the medical group team and third party ROI vendor to promote a harmonious work environment.

  • Reviews HIM workbaskets in Indexing application, addressing issues and routing as appropriate.

  • Runs reports from indexing application and provides feedback to practices regarding scanning/indexing issues as appropriate

  • Participates in meetings with the systems medical record vendors to confirm the medical group needs are being met.

  • The HIMTech II will work in multiple electronic systems throughout the day handling items related to document correction and encounter completion.
