Catholic Health Initiatives Case Management Coordinator RN in Oakes, North Dakota

Overview

As our Case Management Coordinator at CHI Oakes Hospital, you’ll work with physicians, other registered nurses, various departments, and community resources.

You'll monitor and evaluate patient care practices in the acute, emergency department, and swing bed units. More specifically, you'll perform case management functions including coordinating a patient’s care across a continuum of care; facilitate positive clinical and cost effective outcomes; and serve as a resource for the medical staff and nursing. You'll thrive in this role while working well clinically and interpersonally with the Medical Providers to support care coordination. This ensures patients are provided with the education and resources needed to build a care plan at discharge.

About Us:

CHI Oakes Hospital is a 20-bed critical access hospital that provides various inpatient and outpatient services to approximately 14000 people in southeastern North Dakota. It is also a 24-Hour Emergency Level V Trauma Center. The hospital building was newly-constructed in 2007 replacing a 50-year old building and in 2010 Oakes Community Clinic was opened within the hospital building. The hospital is part of a larger values-based organization CommonSpirit Health.

Responsibilities

  • Coordinates discharge plans with the discharge planning team.

  • Documents discharge planning as an ongoing review.

  • Monitors and coordinates swing bed referrals.

  • Assists with developing complete care plans to assist with the patient's needs.

  • Assists as needed with obtaining referrals, prior authorization for Home Health Care, DME, SNF, etc.

  • Acts as a patient advocate and assists with problem solving and addressing any barriers to care or compliance with care plans.

  • Provide referrals to appropriate community resources; facilitate access and communication when multiple services are involved; monitor activities to ensure that services are actually being delivered and meet the needs of the patient.

  • Facilitate disease prevention and health promotion with patients and families.

  • Promote chronic disease management concepts, health screening and preventive health initiatives for targeted patients

  • Follow case and quality management processes in compliance with regulatory guidelines and company procedures.

  • Other duties as assigned.

Qualifications

  • Graduate of an accredited school of nursing.

  • Bachelor of Science in Nursing (BSN).

  • Active Registered Nurse License in the state of North Dakota, or compact license eligible to work in the state of North Dakota.

  • American Heart Association (AHA) Basic Life Support (BLS)

Preferred:

  • 3-5 year’s current clinical experience in acute care settings.

  • Prior Case management/ Utilization Review experience preferred.

Pay Range

$27.70 - $40.17 /hour
