Chenango County Outreach Worker in Norwich, New York

Chenango County's Area Agency on Aging department has an opening for an Outreach Worker. The work involves providing a variety of support services to clients in the areas of outreach, information and referral. An employee in this class identifies clients' needs for services through face to face or telephone contacts and provides information to clients about appropriate services, benefits or entitlement programs. Responsibilities also include assisting clients in accessing services and may involve intervention, negotiation and advocacy with providers on a client's behalf to ensure the delivery of needed services and benefits. An extensive amount of travel using the employee's personal vehicle is required to fulfill the duties of this position. Work is performed under the general supervision of the Deputy Director Office of Aging with leeway allowed for the exercise of independent judgment in carrying out the details of the work. Supervision over the work of others is not a responsibility of employees in this class. Does related work as required.<p>

MINIMUM QUALIFICATIONS: <br>(A) Possession of a Bachelor's degree; <br>(B) License and current registration as a Registered Professional Nurse in NYS and 1 year of full-time paid nursing experience;<br> (C)Possession of an Associates degree and 2 years of full-time paid experience in social casework, social work in a community or social action program, teaching in an accredited school, or as a community services worker in case aid in a human services agency;<br> (D) Graduation from high school or possession of high school equivalence diploma and 4 years of experience as in (C).<p> Special Requirement: At time of appointment and continuing throughout employment, a candidate/incumbent must possess a valid New York State Driver's License, a reliable vehicle, and a driving record acceptable to the County's insurance carrier. <p> A civil service examination will be required for permanent appointment at a later date.<p>
