County of Montgomery Community Connections Supervisor in Norristown, Pennsylvania

The Community Connections Supervisor - Outreach and Communication Coordinator has 2 primary functions: responsible for the coordination and delivery of services performed by the Community Connections Navicate staff to ensure coverage of all offices and coordination of the Office's community outreach activities.  Activities include creating and maintain a communication plan, scheduling staff attendance at community events and collaborating with other HHS offices and community agencies for regular staff education of resources available to the community.  The Community Connections Supervisor may occasionally need to provide office coverage and attends events on nights or weekends.

  • Provides monthly supervision of staff, monitors performance and provides feedback via 6 month and annual evaluations, provides performance coaching, progressive discipline as necessary, and support around challenging client situations. Travel to 3 regional offices to provide on-site supervision is required monthly, at least. 
  • Collaborates with management team to ensure coverage of offices and clients requests for Navicate assistance are being responded to timely.
  • Occasionally required to perform the role of a Navicate to ensure office coverage.   This includes engaging clients and providing accurate information and referrals based upon identified needs and documenting according to the office protocols.
  • Conducts monthly auditing of documentation to ensure staff are documenting in accordance with the Office's policies including a standardized screening, assessment process, and provider record management.
  • Ensures staff comply with County, HHS, and Office's training requirements. Suggests training opportunities for staff to promote professional growth.
  • Secures presentations with County offices and community organizations for staff meetings to maintain staff knowledge of current resources and possible programmatic changes.
  • Becomes certified as a Person-Centered Counselor through PA LINK. Reviews PCC assessment documentation completed by Navicate staff, in county client management system. Works to ensure all eligible clients are being enrolled and documentation is completed timely and accurately to ensure compliance with funder. 
  • Ensures Navicates have current information and literature in each office easily accessible to the Navicate and visitors. 
  • Oversees and updates the Office's Communications Plan (quarterly, annually) which outlines the Office's strategies to create and maintain presence in the community. Includes goals for staff participation in community events, website updates, social media content, and development/distribution/supply of all promotional materials.
  • Supervisor and staff rotate attendance at County collaboratives, task force/HUB meetings, and stakeholder groups. Information gathered is shared with staff to enhance their knowledge base, develop and maintain relationships, and improve referrals to the community.
  • Outreach Coordination requires collaboration with the Office of Community Information to ensure representation of Health and Human Services staff at various community events such as health fairs and expos.  This requires scheduling of Navicate staff or self to attend, collecting materials and supplies for the day of the event while ensuring adequate coverage of offices. 
  • Prepares and conducts in-person and virtual presentations to the community and other county offices monthly or as requested.  This includes creating a Power Point slide presentation, scheduling the presentation and occasionally collaborating with a Navicate to present with the supervisor.
  • The supervisor will work with community organizations to assess and pilot on-site delivery of Navicate services ("Expansion Sites").  The supervisor will manage the Expansion Site activities in the county database and monitor staff documentation compliance in accordance with office's documentation requirements. Provides reports to Administrator (typically on an annual basis) including but not limited to the following information: number of clients served, provider referrals, type of assistance needed, basic demographic information on clients, number of Person-Centered Counseling contacts completed. Analyzes the data to report performance measures and outcomes of the Office.
  • Assists Administrator in establishing and implementing standard operating policies.

  • Responds to direction from Administrator or other County/HHS Leadership so that the office can fulfill the agency's mission.
  • Performs other duties as required to maintain office functions.

  • BA/BS in Human Services, Social Work or related field *AND *5+ year's relevant experience.  If less than 5 years' work experience, a master's degree in human services, Social Work or a related field would be considered.
  • Ability to deliver services with an understanding of a wide array of others' personal preferences, choices, values, belief systems, cultural and language differences.  
  • Flexible, team player who thrives in a collaborative environment. Excellent oral and written communication skills.  Excellent time-management skills.
  • Must possess a valid state driver's license as there will be travel throughout Montgomery County.

Highly Desirable Skills

  • Previous supervisory experience in the field. 
  • Experience working with/using social media platforms.
  •  Knowledge of human services delivery systems in Montgomery County and assisting clients in connecting with services.  
  • Ability to work well with community partners and establish ongoing relationships with providers.
  • Previous training or certification in trauma informed care and recovery principles. 
  • Experience working with client databases, Microsoft Office Suite, virtual platforms and other office equipment.
  • Bilingual abilities are high desirable.
