PruittHealth Director of Dementia Care Services in NORCROSS, Georgia

POSITION SUMMARY:

The Director of Dementia Care Services provides quality support service, as a member of the operations team and is responsible for the development of the Dementia Care Services program for Pruitt Healthcare Centers in accordance with policies and procedures and regulatory agency requirements to assure that the highest degree of quality patient care and customer service. He/she provides education, mentoring, and validation of the PruittHealth Dementia Care Services Program for each center with a Memory Support Unit and other key customers including ongoing analysis of clinical operation outcomes to determine priority needs/opportunities. The Director of Dementia Care Services recommends utilization of resources, environmental modifications, activity programming, and on-going education in an efficient and economical manner to enable each center to achieve and sustain compliance with all applicable regulations and quality outcomes.

ESSENTIAL JOB FUNCTIONS:

  1. Provides professional expertise as a member of the operations team when planning, designing, developing, and communicating the implementation strategies for PruittHealth’s Dementia Care Services program. Critically analyzes organizational issues after a review of evidence to offer quality improvement plans. Applies clinical theories and trends appropriately in an educational/clinical setting.

  2. Serve as a change agent, assisting others in understanding the importance, necessity, impact and process of change while providing leadership and guidance to all partners in formulating the goals and objectives of the program for their centers.

  3. Develops and presents educational workshops utilizing evidence based best practices on a variety of patient care topics. Engage facility leadership and others in decision-making and promote decisions that are patient-centered. Uses personal qualities and professional techniques to stimulate the interest of learners, motivate them to constructive action and incorporate knowledge and skills. Analyzes and utilizes evaluation feedback to assist in the revision and development of current and/or future workshops and conferences.

  4. Develops and publishes educational programs for online learning in Pruitt University.

  5. Provide consultation to Regional Teams, Administrators, Director of Health Services, and Consulting Services Consultants regarding Dementia Care program and patient care best practices.

  6. Collaborates with Internal and External Expert panels, Medical Director(s) and others to develop patient care protocols, policies and procedures.

  7. Utilizes established Quality Measures (QMs) as target outcomes to measure success and identify specific training needs. Articulates patient care standards as published by of state and federal laws and regulations, JCAHO and literature that apply to the practice of long term care.

  8. Maintains knowledge of current clinical practice and the roles and functions of patient care team members. Applies comprehensive knowledge of behavioral processes, standards of care, and demonstrates sound decision-making and judgment in educational activities involving all levels of patient care personnel.

  9. Represents clinical operations at professional clinical services organizations and consults with industry experts for current state or practice and trends in the provision of patient/resident care and services. Attends corporate conferences/seminars for continuing education.

  10. Functions as a company leader and change agent by providing feedback to Supervisor and Director of Policy Management regarding policies & procedures, forms, initiatives, etc. Assures, once policies/ procedures, materials or initiatives are finalized, that implementation is completed at each center. Assists center staff and peers in understanding and positively embracing company goals and strategies for success.

ESSENTIAL SKILLS AND EXPERIENCE:

Minimum of three years of professional nursing, rehabilitation or social services experience.

Must have additional behavioral management or dementia care clinical and education experience, or any equivalent combination of experience and training which provides the required knowledge, skills, and abilities.

Basic competency in E-mail, Word, Power Point, Excel, and internet programs are required. Effective Oral Presentation skills as well as curriculum and course design ability.

PREFERRED SKILLS AND EXPERIENCE:

• Exceptional communication skills with peers, professional staff, consultants & patients in interdisciplinary care setting and government agencies.

• Knowledge of emergency and disaster preparedness.

• Ability to travel and work independently.

• Exceptional knowledge of quality assurance and performance improvement and data analytics.

LICENSURE, CERTIFICATION AND EDUCATION REQUIREMENTS:

RN or Licensed Therapist or MSW; Graduate of accredited school with a minimal of a Bachelor Degree

Computer, phone systems, copiers, fax machines and various other office equipment.

Tasks assigned to this position may involve potential and/or direct exposure to blood, body fluids, infectious disease, air contaminants, and hazardous chemicals. May be subject to hostile and emotionally upset patients/residents/clients, family members, associates, and visitors.
