Organization Overview:The Goddard Riverside Community Center (Goddard Riverside) and the Stanley M. Isaacs Neighborhood Center (Isaacs Center) are well-established Manhattan community-based agencies that support services to approximately 30,000 New Yorkers. Purpose of Position: The purpose of the Case Manager position will be to join a team of social workers to provide social services that enhance the clients’ overall quality of life and increase their access to a variety of resources. Our focus is on financial stability, health and wellness, food, and housing security. The ideal candidate will possess strong interpersonal skills to facilitate collaboration with the team and together deliver impactful results.

Schedule:Monday through Friday 35 hours per week

Salary Range: $40,200 to $50,247

Candidates must have the required qualifications to be considered for the position. The salary offer will be based on the candidate’s number of years experience in the exact or comparable position and additional preferred qualifications will be taken into account.

Qualifications

  • Assist and advocate for older adults by providing support with a wide range of issues, including benefits, utilities, home care and assorted resourcesDocument the assistance provided to clients and maintain a daily log of such services

  • Assist the team with program related social, educational, and cultural activities provided in group settings

  • Provide ongoing reassurance calls to homebound clients or isolated clients

  • Perform outreach activities such as outreach calls, surveys, and communication of relevant information regarding building activities and community events

  • Conduct initial intakes and update clients’ charts as needed when changes take place. (change of insurance, phone number, emergency contact information)

  • Assist clients to make a smooth transition into Phelps House when they move in by helping them with the following tasks (change of address form, assorted benefits and health insurance notification of new address, utilities’ installation and change of address)

Phelps House Team Member

  • Attend staff meetings, supervision sessions, mandatory trainings, and case conferences

  • Participate in program assessment and improvement plans

Qualifications/Educational Requirements

  • Bachelor’s degree in social work, human services, or related field or

  • Associate degree in social work, human services or related field and at least two years of experience working with an older adult population

  • Bilingual English/Spanish required

  • Computer skills (Word, Excel)

  • Strong writing and verbal communication skills

  • Prior experience working with older adults preferred
