Organization Overview: The Goddard Riverside Community Center (Goddard Riverside) and the Stanley M. Isaacs Neighborhood Center (Isaacs Center) are well-established Manhattan community-based agencies that support services to approximately 30,000 New Yorkers. Position Summary: The Home Delivered Meals Case Management Unit serves homebound older adults in Upper Manhattan and Roosevelt Island and is searching for Case Managers for our team who will provide key client support. Case Managers will assess the needs of older adults, provide information on appropriate services and programs, and make referrals for services as needed. They will also assist with the collection of documents, complete necessary paperwork for benefits, and provide linkages with other service providers. The Case Managers will work directly with the Case Management Unit Program Director, Supervisors, and Intake Coordinator at Carter Burden Network, and interface with organization staff and partners.

Schedule:Monday through Friday, 35 hours per week

Salary Range:$54,000- $60,000

Candidates must have the required qualifications to be considered for the position. The salary offer will be based on the candidate’s number of years experience in the exact or comparable position and additional preferred qualifications will be taken into account.

Roles, Responsibilities, and Essential Duties

• Maintain an ongoing caseload of 65 clients and maintain appropriate contact as per NYC Department for the Aging requirements • Conduct comprehensive assessments of clients, develop care plans, and work with clients, families, caregivers, and other professionals in coordinating the care plan. This will include, but not be limited to, coordination of entitlements, referral for supportive services, advocacy and follow-up regarding additional needed services, and ongoing monitoring and reassessment of care with modifications in plan. • Complete approximately 1,500 case management hours per year • Maintain compliance with NYC Aging regulations regarding service provision, deadlines, and documentation • Utilize an online platform to maintain detailed client records including case notes, referrals, and client assessments • Participate in weekly and agency-wide staff meetings, agency events, in-service trainings and conferences • Participate in case conferences as an advocate for clients as needed • Attend the required number of trainings as outlined by NYC Aging standards • Meet with the Case Management Unit and Supervisor • Exhibit awareness of agency and community resources and effectively utilize both in the course of working with staff and clients • Make referrals to relevant organizations and agencies to assist clients and improve overall quality of life• Initiate and maintain contact with representatives of the community and government agencies on behalf of clients• Provide crisis intervention as needed • Exhibit ability to work well within an agency and engage appropriately with co-workers within and outside of the Case Management Unit• Accept other assignments, as appropriate, to meet the needs of the program and individual clients

Qualifications/Educational Requirements

  • The Case Manager should have an affinity for working with older adults, have strong interpersonal skills, and work well with clients and colleagues. They should be good communicators with strong written skills, attention to detail, and a facility with data input. The Case Managers should have the capacity to manage a caseload, meet deadlines, and travel, as necessary, to meet with clients in their homes on the Upper East Side, Roosevelt Island, and/or East Harlem

  • Bachelor’s degree with 1 year experience.

  • Spanish or Mandarin fluency is highly desirable.

    Skills/Knowledge/Abilities

    • Proficient in Microsoft Word, Excel, Outlook and Publisher. • CPR certified and AED/First Aid Certified or be willing to attend both trainings• Bilingual, English/Spanish or English/Chinese highly preferred. • Highly resourceful team player, with the ability to also be extremely effective independently • Bicultural skills desirable, but not required• Ability to work some flexible hours (e.g., evenings and weekends)
