Cornell Scott Hill Health Corporation Site Lead Registered Nurse in New Haven, Connecticut
The Site Lead Registered Nurse is responsible for guiding and supervising assigned nursing personnel in delivering patient care at our South Central Rehabilitation Center in New Haven, CT. The Site Lead RN will collaborate with nurses, physicians, and other health care professionals in the delivery of patient care. Assumes responsibility for management of a selected case load of patients as defined by the clinical practice area. The Site Lead RN is additionally responsible for assisting the Departmental Nurse Manager with essential leadership functions such as scheduling, payroll, and day-to-day management of the clinical operation.

Site Lead Specific Responsibilities:
Under the leadership of the Departmental Nurse Manager, works with the Registered Nurses (RNs), Licensed Practical Nurses (LPN) and Medical Assistants (MA), at designated CSHHC site, while coordinating with other CSHHC sites, to ensure adequate staffing.
Assists in recruitment with Nurse Manager and helps with orientation, training and retention of new staff (LPN’s and MA’s) and the maintenance of their competency.
Assists Clinical Nurse Educator with orientation, training and retention of new support staff (RNs, LPN’s and MA’s).
Works with providers and Nurse Manager to develop and/or implement clinical protocols for nursing visits.
Identifies Quality Improvement (QI) issues and leads QI projects for designated CSHHC site, while working with QA/QI Nurse to assure staff meets all regulatory and patient care standards.
Partners with site administrators and front desk colleagues to identify and develop practice and patient experience improvement projects.
Actively participates in panel management as part of the clinical care team through proposing, implementing and/or monitoring standing orders approved by the institution and/or clinical leadership.
Performs all other duties as required of the position and requested by leadership team.
Registered Nurse Responsibilities:
Performs assessments, data collection and modifies patient care in an ongoing and systemic manner, focusing on urgency including physiological, psychological, psychosocial and cognitive status of patients based upon symptoms and department standards of care.
Responsible for performing in person and phone triage.
Performs complete documentation responsibilities relating to patient's medical records in a timely, accurate and concise manner. Assesses and documents conditions, to include skin integrity and vital signs.
Ensure patient's rights are upheld as by the compliance committee and officer.
Accurately recognizes abnormal diagnostic values for the patient, according to age specific guidelines. Interprets their significance and follows through with the provider.
Formulates patient specific goal-directed plan of care which is prioritized and based on nursing assessment and patient outcomes, including seeking participation of patient and significant others.
Explains rationales for all nursing, technical and therapeutic interventions selected and conducted. Demonstrates knowledge of the relationship of pathophysiology to procedures and interventions.
Performs all other duties as required of the position and requested by leadership team.
Requirements:
Graduate from an accredited school. Must have a current Registered Nurse Connecticut State Licensure. CPR certification required.
Minimum of five (5) years’ experience in a long-term care facility, rehabilitation center, ICU, emergency department, or Department of Corrections. Clinic experience is highly desirable.
Excellent Patient Management skills.
Must be strongly motivated and able to accept supervision.
Computer skills are essential with the ability to learn EMR and EPIC medical records.
Ability to provide phone triage along with knowledge of referral process, medication refill procedures and administration.
Knowledge of CLIA waived lab testing, Urine Dipsticks and Glucometers are necessary.
Excellent leadership qualities and knowledge of management skills are necessary.
