**Purpose Under the direction of the Director of Emergency Services, the Recovery House and Respite Manager, provides case management to Medical Respite and Recovery House residents. The manager will coordinate care with the YNHH and VA Hospitals to accept appropriate referrals; networks with Greater New Haven shelter system and mental health/substance abuse/social/medical/housing service systems and collaborates with clinicians and other service providers as well as other agency case managers to empower clients to end their homelessness. The manager also provides short term case management services to adult males leaving a detoxification center and/or waiting for or just leaving an inpatient treatment facility.
