Summary The Clinical Navigator is primarily responsible for the support and education of end-users, caregivers, and clinicians with Atos products and services in the Case Management Program. The Clinical Navigator works with the clinician in collaboration with the Atos Clinical Educators and Territory Sales Managers inquiries and concerns to assess, facilitate. plan and coordinate integrated product utilization for the end user. The navigator will develop and implement an individualized care plan in collaboration with the patient or patient\'s support network to address the patient\'s specific product needs and utilization to reach mutually established goals over time. The Navigator will work to ensure that the patient is making progress toward their desired outcomes. Essential Functions Completes comprehensive assessments of members per regulated timelines and determines who may qualify for case management based on clinical judgment, changes in member\'s health or psychosocial wellness, and triggers identified in the assessment. Develops and implements an individualized case management plan in collaboration with the patient, caregiver, physician and/or other appropriate healthcare professionals and member\'s support network to address the member needs and goals.
