Job Summary and Qualifications

The Case Manager Team Lead is responsible for supporting the Director Case Management and Manager of Social Work in managing the Social Work operations. This includes implementation and enforcement of policies and procedures, supervision and orienting/training of staff, quality assurance and regulatory compliance. Additional duties may include assisting manger with payroll, educational material and supportive clerical duties. May delegate responsibilities and duties to other personnel, both professional and non-professional. Will assist Director and Manager to coordinate Social Work activities with other departments.

What you will do in this role:

• Assists the Manager of Social Work in providing leadership and direction for the department based on hospital strategic plan and planning, organizing, and directing.

• Assists with reviewing productivity via PLUS at a frequency to allow timely staffing decisions to achieve the least costly staffing mix and minimizing overtime.

• Maintains flexibility in order to fill staffing gaps in work schedule.

• Oversees daily function to ensure department policies, procedures, protocols, regulations, and standards are followed by self and staff.

• Coordinates departmental PI program, outcomes management projects and other hospital initiatives as directed by manager.

• Assists with new hire orientation and preceptorship.

• Assists with and provides input to the manager for employee performance management plans.

• Coordinates and integrates social work services within the department and with other departments.

• Mentors students, volunteers, staff and physicians on clinical issues and serves as a positive example to employees, patients, families.

• Provides support to Social Work staff in absence of Manager/Director

• Be able to identify and address specific needs of homelessness, substance abuse, domestic violence, elder abuse or vulnerable persons, suspected child abuse/neglect, substance abused newborns, teenage pregnancy, accidental ingestion, suicide attempts, etc.

• Provide training, bereavement intervention and education to Nursing residents/physician residents, nursing, volunteers and support staff

What qualifications you will need:

• Masters Degree

• State license preferred (LMSW or LCSW or LAPSW)

• 2-3 years’ experience in an acute care medical setting

• Knowledgeable about internal, community and post-acute care resources

• Basic Social Work skills including but not limited to empathy, sympathy, sensitivity to cultural and ethnic diversity, advocacy, communication and crisis intervention skills
