Covenant Health Licensed Nurse Assistant-LNA-per diem - Day Shift - Med-Surg in Nashua, New Hampshire

Summary

Renders nursing care to patients within an assigned unit of the hospital, in support of medical care as directed by medical staff and pursuant to objectives and policies of the hospital

Essential Duties and Responsibilities

  • Supports and promotes the mission and values of Covenant Health Ministry.

  • Practices according to the ANA guidelines of care as adopted by the Nursing Division

  • Adheres to the Nurse Practice Act of NH.

  • Performs nursing techniques for the comfort and well-being of the patient.

  • Reports changes assessed in patient condition to Registered Nurse.

  • Communicates changes in patient condition to the charge RN

  • Follows physician orders and incorporates interventions in the Nursing Care Plan.

  • Expresses interest in the patient and their progress assuring education and discharge needs are met.

  • Prepares equipment and assists physicians during treatments and examination of patients.

  • Administers prescribed medications, orally and by injection, according to safe practice guidelines.

  • Uses equipment/safety protocols/and policies in providing treatment to patients.

  • Observes patient response to medication and treatment and reevaluates as needed.

  • Incorporates patient psychosocial, spiritual, and cultural beliefs into the plan of care.

  • Demonstrates effective verbal and non-verbal communication techniques.

  • Communicates written messages in accurate, appropriate, and legible format to include patient’s condition; reaction to drugs; treatments: patient response to interventions/treatments and significant changes in patient condition.

  • Explains procedures and treatments ordered to involve the patient in decision making in their care.

  • Promotes a positive work environment each shift

  • Participates in daily unit huddles for safety

  • Records nursing needs of patients on nursing care plan to assure continuity of care.

  • Active membership with Unit Advisory Council.

  • Assists with hourly rounding for patient safety

  • Utilizes communication techniques, such as AIDET, aimed at improving patient satisfaction

  • Active participant with unit-based quality improvement initiatives aimed at improving patient outcomes.

  • Remains up to date with unit-based quality and infection prevention data (CAUTI, CLABSI, CDIFF, HAPU, FALLS, HH and VAE).

  • Completes all mandatory learning within the timeframe allocated.

  • Participates in monthly skin prevalence survey

  • Avoids use of unauthorized overtime

  • Manages ETO (Earned Time Off) per policy

  • Other duties as consistent with this role.

    Job Requirements

    Job Knowledge and Skills

  • Crisis Prevention Institute (CPI) training preferred or obtained prior to completion of orientation

  • IV certification required or obtained within 90 days.

    Education and Experience

  • Current New Hampshire LPN license required

  • Basic Cardiac Life Support required

  • Graduate of high school or GED preferred

  • Graduate from an approved School of Nursing preferred

    An equivalent combination of education and experience which provides proficiency in the areas of responsibility listed above may be substituted for the above education and experience requirements.

    Working Conditions/Physical Demands

    Must possess the physical and mental abilities to perform the tasks normally associated with this position that involves a combination of sitting, standing, walking, bending, stooping, and reaching. Lifting/carrying frequently up to 35 pounds, and occasionally up to 50 pounds. Some stress related to high level of responsibility for quality care.
