Description:

Community Health Worker (CHW) position whose primary function is to provide navigational and enrollment services to support individuals within the community in overcoming the Social Influencers Of Health (SIOH) barriers.

Position Summary:

This culturally and geographically connected individual serves as a link between underserved communities and existing community resources. Through outreach, home visiting, familiarity with local health and human service providers and advocacy, this individual assists clients in overcoming barriers to accessing healthcare, social services, education, and employment.

What you will do:

  • Enrolls clients in available federal, state and local programs to increase access to health care, healthy food and social service programs.

  • Conducts home visits as outlined in the Health Project’s health navigation program.

  • Conducts outreach efforts to community and neighborhood organizations to promote the availability of Health Project Programs.

  • Conducts client eligibility interviews and makes appropriate referrals to other services and programs.

  • Manages client records and inputs necessary information into designated information systems in a timely manner.

What you need:

  • Fluent in Spanish and English preferred

  • Community Health Worker Certified, preferred.

  • Two years of experience in outreach and enrollment work is preferred. Familiarity with local health and human resources

  • GED/high school diploma required
