Description:

Client Services Coordinator (CSC) is the provision of a range of client-centered activities focused on improving health outcomes in support of the HIV care continuum for low acuity clients and those with access barriers to transportation and food. The Client Services Coordinator is a professional who collaborates with physicians, health care team members and patients to support coordination of care for low acuity patient populations across the continuum with specific and intermittent barriers to care and basic needs. Provides various non-medical case management services to meet needs of patients including healthcare navigation, accessing local resources, provision of medical transportation and food pantry delivery. In collaboration with CSC Lead, supports addressing gaps identified in the patient care plan, and linkage to local resources necessary for patient engagement in care. Ensures patients access to health and support services are reduced through direct medical transportation and food pantry delivery.

Position responsibilities:

1. Delivers Food Pantry as scheduled ensuring adherence to grant spending and distribution guidelines:

• Assists in taking pantry orders as needed.

• Delivers pantry to patient home or designated location.

• Appropriate documentation of all pantry deliveries

• Assists with activities related to restocking pantry as needed.

• Reports concerns to manager and engages in problem solving.

1. Provides direct Medical Transportation ensuring adherence to grant guidelines:

• Direct transportation in Dept. Fleet Vehicles as scheduled.

• Appropriate documentation and requests to leaders of any/all off cycle fleet vehicle service needs.

• Ensures Fleet vehicles gas tanks are full at the end of each week.

• Appropriate documentation of direct transportation Services

• Reports concerns to manager and engages in problem solving.

2. Collaborates with Lead, manager, colleagues and Quality Mgt. on policy/process improvement. Includes but is not limited to support and guidance around grant requirements, funded activities, performance measures and quality management/improvement activities for NMCM, Medical Transportation and Pantry services.

3. Provides various direct services including: coordination of services based on identified needs, referrals to community services as necessary to assist the patient in transitioning along the health care continuum.

4. Provide ongoing education to clients on identified needs including treatment adherence and risk reduction, community resources, healthcare navigation and insurance navigation.

5. Consistently communicates in a clear, accurate, direct, open and timely manner with all customers-patients, family, physicians and nursing staff.

6. Demonstrates an ability to interact professionally and effectively with team members to ensure a collaborative plan of services. Keeps manager and staff informed about relevant issues.

Position Qualifications:

High School degree or equivalent. Bachelor’s degree in Health or Human Services preferred. Prior employment experience in a related area preferred. Demonstrate ability and skill in the management of patient care planning and continuous quality improvement in services delivered. Demonstrated ability to engage in consistent communication with members of the patient care team; participation in case conference or other collaborative meetings where decisions about patient care are taking place.
