This role is a professional RN with service line specific clinical knowledge who offers individualized assistance across the care continuum to patients, families, and caregivers to expedite and coordinate care, and address health system barriers. Assumes responsibility for the coordination of patient care through interdisciplinary and multidisciplinary collaboration to achieve optimal patient outcomes with a focus on high-risk patients or those with complex care needs (including multidisciplinary care). This role has a particular focus on improving transitions in care and adherence to care and functions as a clinical advocate and educator for patients.

MINIMUM QUALIFICATIONS: EDUCATION, CERTIFICATION, AND/OR LICENSURE: 1. Current unencumbered licensure with the WV Board of Registered Professional Nurses, or appropriate state board where services will be provided, as a registered professional nurseORCurrent multi-state licensure through the enhanced Nurse Licensure Compact (eNLC). 2. Obtain certification in Basic Life Support within 30 days of hire date. EXPERIENCE: 1. Three (3) years of nursing experience.

Assesses and facilitates coordination of multidisciplinary care across the care continuum. Provides communication, advocacy, and education in a culturally competent manner. 2.Clinically triages new patient referrals efficiently, to ensure timely access to care. 3. Collaborates with multidisciplinary care team and expedite sequence of patient's workup, active treatment, survivorship, and end of life care. 4. Provides communication and reinforcement of clinical information and serve as a conduit between patient and providers/clinicians to address needs of patients. 5. Performs targeted symptom assessment and management for identified patient population. 6. Assesses health literacy and educational needs of patient. Provides clinical education about diagnosis, treatment, side effects, and post- treatment care. 7. Assesses and documents patient's needs, goals, and preferences and work to ensure such are integrated into treatment and care delivery. 8. Bridges gaps in care and assesses/troubleshoots care transitions and barriers to care. 9. Documents all patient encounters in EPIC. 10.
