Job Title: Physical Therapist

Ministry Name: Trinity Health PACE Mercy LIFE Alabama

Hours: PRN

Position Purpose: Trinity Health PACE seeks a dedicated Physical Therapist to deliver clinical evaluations and treatment in acute, restorative, or maintenance settings across our Day Center, Nursing Home, or in-home environments. Your role is pivotal in helping participants achieve optimal functional independence and well-being.

What You Will Do:

  • Conduct comprehensive physical therapy assessments and develop individualized care plans.

  • Provide therapeutic interventions to participants with diverse disabilities, promoting maximum functional independence.

  • Collaborate with interdisciplinary teams and assist in managing durable medical equipment needs.

  • Train staff in safe body mechanics and use of therapy equipment.

  • Maintain high standards of documentation, safety, and quality assurance.

Minimum Qualifications:

  • Bachelor's degree in Physical Therapy from an accredited program.

  • Active, unencumbered physical therapy license in the state of North Carolina.

  • At least one year of experience working with frail or elderly populations; less experience requires PACE-specific training.

  • CPR and First Aid certification may be required.

  • Demonstrated ability to work independently, manage conflict, and communicate effectively with diverse groups.
