Care Manager LCSW at Holy Cross Medical Center in Mission Hills, CA. This is a PER DIEM/Day shift position that will work 8 hour shifts.

The Licensed Clinical Social Worker (LCSW) provides biopsychosocial assessment, treatment planning, and implementation of social work services to patients and families along the continuum of care. The Clinical Social Worker utilizes his/her knowledge and clinical skill sets to address needs of vulnerable populations and in complex discharge planning situations. In addition to addressing barriers to coping with illness or injury, the LCSW has a specialized role with regard to assessment and intervention of abuse, suicide risk, and other mental health issues. The LCSW demonstrates proficiency in delivering care to assigned age specific populations (e.g., infants, pediatrics, adolescents, adults, and geriatrics).

Required Qualifications:

• Master's Degree in Social Work from a CSWE university.

• California Clinical Social Worker License upon hire.

• California Fire and Life Safety Card - National Organization within 30 days of hire.

• 1 year of Clinical experience in a healthcare setting.
