Fairview Health Services Social Worker - MSW in Minneapolis, Minnesota

Overview

We have an opening for a .9 pediatric clinical social work position. This role follows the pediatric solid tumor patient population including: Osteosarcoma, Ewing’s Sarcoma, Rhabdomyosarcoma, Neuroblastoma, Hepatoblastoma, Wilm’s Tumor, Germ Cell Tumors, Other Sarcoma(s). The clinical social worker is considered the primary mental health provider on the interdisciplinary team. The Solid Tumor Social Worker will follow these patients and families through the continuum of care/setting including initial diagnosis, throughout duration of treatment (6+/- months to 2 + years) and until they either transition to long term survivorship after 5 years or pass away. 

Reaasons for social work intervention may include:

  • New diagnosis 

  • Relapse/progression of disease 

  • Family care conferences 

  • Change in treatment plan 

  • End of life care and Palliative care  

  • Child protection issue/security concern  

  • Emotional needs (adjustment, coping, emotional support) 

  • Crisis intervention 

  • Mental health assessment  

  • Financial/insurance/legal concerns  

  • Transportation concerns 

  • Housing/lodging concerns 

Responsibilities Job Description

  • Support the patient and family’s coping and emotional needs. 

  • Promote adaptation to illness and treatment  

  • Help families navigate through the medical system, bridging the connections between the family and the Hem/Onc interdisciplinary team. 

  • Provide counseling to individuals, siblings, couples and families  

  • Teach relaxation techniques, communication and coping skills 

  • Help with problem solving, advocacy and communication about the patient’s plan of care 

  • Help with school issues (arranging for home and hospital teaching, advocating for IEP and 504 educational plans, and guidance with school re-entry) 

  • Help identify hospital and community-based resources: emotional, supportive and/or financial 

Qualifications

LGSW/LICSW

Previous healthcare experience preferred.

Clinical supervision is provided.
