JOB REQUIREMENTS: Position Description: The Medical College of Wisconsin\'s Department of Medicine, Division of Pulmonary and Critical Care is seeking a Nurse Practitioner or Physician Assistant to provide primary or specialty medical care within defined scope of practice under the supervision of a licensed physician and in accordance with MCW/FMLH/CHW policies and procedures. This position will provide care focusing on the Lung Transplant patients in an outpatient and inpatient setting located at Milwaukee, WI at Froedtert Hospital - Center for Advance Care. The Advanced Practice Provider (APP) works as a member of a multidisciplinary health care team who will contribute to excellence in program administration, patient care, patient safety, research, quality improvement, and teaching. The Ideal Candidate will be a certified and licensed Nurse Practitioner or Physician Assistant with women\'s health experience wanting to be part of a thriving and very collaborative academic practice at the Medical College of Wisconsin (MCW). Areas of responsibility will include, but not be limited to providing direct patient care in an outpatient setting. In this role you will: Collaborates with interdisciplinary health care teams in diagnosis and treatment of conditions as a member of the Lung Transplant service. Writes accurate and timely progress notes, procedure notes, and/or admission and transfer orders. Order, perform, and interpret screening and diagnostic procedures and tests, and appropriately communicates and documents information. Privileged to perform procedures beyond core as identified by the Lung Transplant Services. Facilitates and communicates with patient, family, and staff to promote continuity of care across the continuum of care. Attend required training and meets key regulatory requirements, including those of the privileging institution, CMS, and the Joint Commission. Evaluates quality outcomes for the patients receiving care of this team through participation in the development of quality indicators and outcomes dashboard. Acquires information on the most current development and trends in the Physician Assistant field and/or Nurse Practitioner field, through educational programs, in-services, professional associates, and current literature with opportunity for continuing education by attending specific national conferences when appropriate. Provide care focusing on the Lung Transplant patients in an outpatient and inpatient setting.
